What is the CMH Foundation?

Founded in 1998, the CMH Foundation is a community-based, tax-exempt organization created to provide donors a way to contribute to the continued growth and enhancement of health care services at Community Memorial Hospital in Hicksville, Ohio.

Funds and their purposes are listed below:

Development Fund

To be used where needed most as directed by the CMH Foundation Board of Directors.

Scholarship Fund

To provide scholarships for students pursuing a career in a medical field.

Endowment Fund

A permanent capital fund that will support the mission of Community Memorial Hospital forever. The principle is not accessible, but the interest is used to benefit Community Memorial Hospital.

Capital/Building Fund

Funds are used to make improvements to hospital and clinic properties, and to purchase equipment.

Ways To Give

Tribute Gifts

A contribution in memory of a deceased friend or relative, or in honor of a special person, doctor, or hospital employee is an especially meaningful tribute.

Planned Gifts

Planned giving provides a method for donors to make gifts to Community Memorial Hospital and also enjoy favorable financial, tax-saving, and income consequences.

You can name the CMH Foundation as a direct or contingency beneficiary of all or a portion of your retirement funds, IRAs, life insurance policies, and other assets.

Annuities and Trusts

A variety of annuity and trust methods can be customized to accomplish your charitable and personal goals. Charitable Gift Annuities, Charitable Remainder Trusts, and Charitable Lead Trusts are three opportunities you may want to discuss with your professional adviser.
