- Assertive Community Treatment (ACT): The purpose of the ACT teams is to deliver comprehensive and effective services to individuals who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. Typically, recipients served by the ACT teams have a serious and persistent psychiatric disorder and a treatment history that has been characterized by frequent use of psychiatric hospitalization and emergency rooms, involvement with the criminal justice system, alcohol/substance abuse, and/or lack of engagement in traditional outpatient services.

- Health Home Care Management (HHCM): The major components of our Health Home Care Management Services have been developed in order to assist recipients to achieve their stated goals and objectives, and to succeed within their community plan of care. These service components include: Intake and Screening,
Assessment and Reassessment, Care Management Planning and Coordination,
Implementation and Ongoing Evaluation of the Care Management Plan, Crisis Assistance and Intervention, Monitoring and Follow-up Services, Family Support and Empowerment, and Community Resource Development.

- Mental Health Outpatient Treatment Program: Experienced social work therapists, psychiatrists and psychiatric nurse practitioners deal with people of all ages – children (6 and older), teenagers, adults and seniors. Our caring staff provides individual, family or group counseling as well as medication therapy.

- Personal Recovery Oriented Services (PROS)These structured, five-day-a-week recovery oriented programs offer services to adults (18 and over). Our objective is to assist members in managing their psychiatric symptoms and to develop the skills necessary to improve emotional, cognitive and social functioning. Treatment will include clinical, case management, psychosocial, co-occurring services and structured skill development. Recovery plans will be developed for each member.

- Residential Services:The goal of our residential programs is to provide a safe and supportive environment allowing our residents to grow and achieve their fullest potentials. Our programs receive funding from The New York State Office of Mental Health (OMH) and the Federal Department of Housing and Urban Development (HUD). We receive ongoing support from the Nassau County Department of Mental Health and the Suffolk County Division of Community Mental Hygiene Services.

- Project FORWARD (Facing Obstacles in Relationships and Work with Action, Resources, Direction). This program provides the following to couples and at risk youth: Prevention education, Skill building workshops, Case management, Economic stability interventions. Project FORWARD participants will experience opportunities to reconnect with their partners, improve family harmony, boost self-esteem, find and keep meaningful employment and greater overall happiness and healthfulness that can transfer to the next generation.

CEO, Jeffrey Friedman, wrote an important piece for Long Island Business News about the urgent need for more timely and accessible mental health and substance use care on Long Island.

Through a powerful collaboration between government and community organizations, our 24/7 Community Crisis Center is helping shift treatment away from delays and stigma toward a new reality of urgency, access, and compassion. 💙

“…where people no longer need to navigate their darkest moments alone. A chapter where help waits for them instead of the other way around.”
