We know these issues pharmacies like us are facing are confusing—in fact, that’s partly intentional on the part of the bad guys (Pharmacy Benefit Managers, or PBMs), since we can’t fix problems we don’t understand. It also doesn't help that PBM abuse is an issues that's inherently backwards---providers paying to care for patients? What's up with that?!

If you’ve already started your self-education on problems in pharmacy but are looking to take a deeper dive into PBM abuse specifically (e.g., who decides what you pay for your medicine and how a pharmacy can lose money filling your prescription), consider checking out our Nurse’s presentation to MNORN! As an independent community pharmacist, I'm reaching out about a critical issue: the survival of independent pharmacies!

Pharmacies face a critical financial situation: Pharmacy Benefit Managers (PBMs) have steadily reduced payments over the years. Now, payments are below the cost of medications, leaving pharmacies unable to cover inventory and other essential expenses. Unless urgent action is taken, one in three independent pharmacies expect to close this year.

Our legislators must act now! Please email your local and national representatives (see link in comments) to urge critical PBM reform. Your voice is essential in preserving community pharmacies and ensuring personalized care for your medical needs. Copy and paste the following message or craft your own:

Meet our founder and learn how her philosophy informs the mission of our Pharmacy.

Dr. Greta Goldshtein is an Advanced Practice Pharmacist and earned her Doctor of Pharmacy degree from USC School of Pharmacy in 1997. She loves her profession and feels that it has allowed her to practice her life philosophy for 20 years and counting, first as a clinical inpatient pharmacist at Cedars Sinai Medical Center and then as an outpatient pharmacist at Roxbury Pharmacy in Beverly Hills. She is passionate about fairness, access, care, support, respect, and realization of potential.

Our Constitution promises “life, liberty, and pursuit of happiness”. Greta believes that these are only possible with societal and personal wellbeing and health. People should enjoy access to good information and engage in consistent and meaningful partnerships with caring, conscientious, and knowledgeable healthcare professionals, all of whom work together toward enabling the best wellbeing and health for every one of us.

Greta has built Courtyard Pharmacy to translate these lofty goals into practice. Pharmacists often serve as educators and coordinators of care, balancing the information, recommendations, and prescriptions from the patient’s multiple physicians and caregivers, screening for appropriateness of care, contraindications, interactions, and potential for side effects. Pharmacists explain therapeutic goals, management decisions, and teach and encourage patients to use their medications correctly.

To actualize her philosophy of healthcare, Greta is building Courtyard Pharmacy as a nexus of patient care: traditional prescription services, over-the-counter medications, vitamins, supplements, and complementary supplies, delivery service, collaboration with healthcare providers, and provision of comprehensive medication management services, immunizations, travel health, in-pharmacy and at-home medication management services.
