Mr. Crosby is a 39 year old father that arrived at our facility following a traumatic motorcycle accident that left him with 13 fractures including his femur, tibia, and humorous fractures, as well as multiple pelvic fractures, rib fractures, and vertebral fractures. Mr. Crosby was not able to bear weight through both legs and his left arm. Due to his many injuries and precautions, progress was slow and painful during the initial month of his stay.

After expressing his desire to get home as soon as possible, Mr. Crosby had a difficult conversation with his therapists about what he would need to do to in order to get there. From that point forward he was out of bed and in the gym as long as he could tolerate, building strength and endurance for when he was cleared to to bear weight through his arms and legs. As his precautions lifted one by one, he made huge leaps in progress every day. In a month and a half, Mr. Crosby has gone from being totally dependent and bed bound to preparing for a discharge home with his family. Seeing his a determination to return home has been an inspiration to all of us at Meridian Center.

"I feel like I got the best care that was available anywhere and I am gratefully appreciative of every single one of you."

Jackie May admitted to the hospital following a fall down the steps at home and was found to have a perprosthetic R hip fracture which had been replaced 10 years prior. On admission Jackie was toe touch weight bearing and unable to transfer to his wheelchair or perform gait. He required max to total assist for all activities of daily living except upper body dressing which was min assist.

From his admission Mr. May expressed his motivation to participate in therapy and return to his prior level which was fully independent in the community and living in his 2 story home. He approached every treatment session with a positive attitude, pushing himself to make progress daily. His goal was to discharge home by his follow up appointment in hopes that he would have his weight bearing status changed.

We hoped for the best, but prepared him for a discharge home regardless of weight bearing status, training him to adapt his ADLs to the first floor of his home at current weight bearing status. By the time he reached his follow up he was modified independent to independent for all ADLs and able to walk with a rolling walker greater than 150 feet while maintaining his toe touch weight bearing status.

Even though the doctor did not upgrade his weight bearing status at his follow up, Jackie May was able to achieve his goal of discharging home the following day due to his hard work, determination, and willingness to adapt and demonstrate safe carry over of his therapy recommendations.

When Mr. Drake arrived at our facility, he was dependent on bed mobility, transfers, and all activities of daily living.

He had previously been living with his brother, but due to the extensive level of assistance he required, his discharge plan was to remain at Meridian Center and transition to LTC. Mr. Drake made slow, steady progress, but was demonstrating poor initiation, and the team experienced frequent barriers with participation. After three weeks, he required minimal assistance with all ADLs and would not attempt walking. During this time, the therapy team, especially Yusuf, developed a strong rapport with Mr. Drake, and through this relationship provided daily encouragement and education. At this point, the IDT team met with Mr. Drake and his family, who agreed that if he could toilet without physical assistance and walk household distances, he could return home.

Yusuf sat down with Mr. Drake to discuss a change in mindset and participation, and from this point forward, Mr. Drake made up his mind to return home. Every morning, he initiated getting ready and rolling himself down to the gym for therapy. He actively participated in exercises, gait training, and ADLs to increase his independence while improving his healthcare literacy in order to better manage his chronic medical conditions. Through his hard work, he is now under supervision for ADLs and walking household distances. During our final meeting with Mr. Drake, we were able to celebrate his hard work and the support of his family that facilitated his discharge home.

Our social work team collaborated with the PT and his siblings to discuss resources in the community to continue his progress, manage his chronic medical conditions, and access support to maintain his sobriety. The holistic approach of the Meridian team and the determination of Mr. Drake to exceed expectations and discharge home has been truly inspiring.❤️

When 38 year old Shane arrived at The Meridian Center following a Left ACA stroke he demonstrated expressive language deficits and used a communication board to communicate with staff, he required moderate to maximum assistance for all activities of daily living and was unable to take steps due to R hemiplegia. We were informed that he was previously living in an Independent Group Home and successfully employed as a cashier. In order to return there he would need to be fully independent with self care and housekeeping tasks and able to navigate 5 steps to enter, or he would need to look at a different level of care.

Throughout his stay, Shane has worked hard to meet and exceed all of his goals in order to meet his personal goal of returning home and living with his friends. Shane is now able to perform all of his ADLs and IADLs independently with use of adaptive equipment, he is walking 125 feet, and is able to walk up 8 steps with a single rail and a cane. Since his admission Shane has come down to the gym daily, advocated for himself, and worked through his barriers with a smile on his face, determined to return home. Shane has touched every member of our team through his resilience, determination and positive attitude. He refused to give up every single day and is able to return home, exceeding the expectations of everyone around him
