Steps to get your Medical Cannabis card in Hawaii (Hawaii 329 card). Currently serving only Hawaii Island (Big Island) patients.

2. Get evaluated by our provider!
must have 1 or more qualifying conditions to qualify for Hawaii 329 card:
Cancer
Glaucoma
Lupus
Epilepsy
Multiple Sclerosis
Rheumatoid Arthritis
Acquired immune deficiency syndrome
Positive status for human immunodeficiency virus (HIV/AIDS)
Post-Traumatic Stress Disorder (PTSD)
-OR-
A chronic or debilitating disease or medical condition or it’s treatment that produces one or more of the following:
Cachexia or wasting syndrome
Severe pain
Severe nausea
Seizures, including those characteristic of epilepsy, or
Severe and persistent muscle spasms, including those characteristic of multiple sclerosis or Crohn’s disease.
Our office will confirm if you have a qualifying condition prior to your evaluation.
All initial evaluations require a face to face visit. Subsequent renewals can be performed face to face or via telemedicine.
3. Complete Hawaii Dept. Of Health Medical Cannabis Registry application.
Can be completed by our office for a nominal fee

4. Receive 329 card via mail or MEDMJ account within a few weeks!
