SPORTS PHYSICAL IMPORTANT INFORMATION!!!
If your Hobart High student has an appointment scheduled for a sports physical, please make sure to do the following
*Call student services & let them know so they can be sent down.
*Make sure Final Forms are completed on the Hobart Athletics website. Print this off and send it with your student.
*If your student us under 18, they must have a Brickie Minor consent on file. They can pick the form up in student services, or in our clinic. Please make sure they bring this to their appointment if they are coming without their parent.
*Sports physicals are 35$. We accept cash and card. Please make sure your child has this for their appointment.
