Continuum Home Health Care

Continuum Home Health Care Why hire just one person to help at home when you can have an entire professional team at your service?

From care planners, to schedulers, to multiple aides, we are all here to make sure your home care service is competent and dependable. Continuum was started in 2019 by Louis Rios, who had a vision and a passion to give the senior population of West Michigan the opportunity to stay in the comfort of their own home with a continuum of quality care. When the time came for my family to make decisions about home care services for my father, we wanted to find the best service in the area. We wanted a company who had the same core values we cherished, skilled workers who were knowledgeable about the health care industry, and service that exceeded our expectations. We struggled to find the quality care that we knew our father deserved, and so I took my father's care into my own hands. I became his primary caregiver and was humbled by my experience. I was able to care for my father up until he passed away. I was fortunate enough to care for many other individuals as well, and I know how rewarding it is to care for someone other than yourself. Our dedication to providing the best care is what makes Continuum Home Health Care a trusted and highly recommended home care agency.

Featured in this post is an actual Creme Brulee cupcake I sacrificed (devoured) yesterday to make sure the second one I bought would be good for todays celebration 🤣

It’s National Gingerbread House Day! 🎄

A perfect reminder to slow down, get creative, and enjoy a little holiday magic.
Building a gingerbread house is more than candy and icing — it’s about memories, laughter, and taking a moment to enjoy the season. ❤️

Share a photo of your gingerbread masterpiece (or mess-terpiece)! 🍬😂

Happy Friday!☕

“Teamwork makes the dream work… and apparently the office chair Grand Prix too.” 🏎️🪑💨

We made it to Friday — and if your coffee is hot and your socks match, you’re already winning. 😂

Whether you're braving the snow, snuggling up inside, or counting down the minutes until the weekend (we see you!), we hope today brings a little warmth and a lot of smiles.

PS “Please note: no office chairs were harmed in the making of this ‘team building exercise.’” 🙃

Stay cozy, stay kind, and have a wonderful weekend! 💙✨

Understanding Medication Management at Home

For many of our clients, managing multiple medications can be confusing. Proper medication management is essential for safety and well-being. Here are some practical tips to share:

🗂️ Keep a Medication List: Write down the name, dose, and time for each medication — keep it visible (on the fridge, in a binder, or a pill-box chart).

📆 Use a Pill Organizer: Weekly pill boxes help prevent missed doses or accidental double-dosing.

📋 Track When Medications Are Taken: Caregivers or family members can note in a log when meds are given — very helpful if there are multiple caregivers.

🔄 Reconcile Medications Regularly: Especially after doctor visits or hospital stays — make sure the list is up to date.

📞 Communicate With Healthcare Providers: If side effects occur or questions arise, don’t hesitate to call the doctor or pharmacist.

Proper medication management helps prevent complications and ensures our clients stay safe and healthy.
