"Hey Doc, my upper front tooth has been hurting me for weeks and no one can figure out why, can you look at it?
This is a case of something called "referred pain", when the site of the pain is not the source of the pain. Look at the panograph x ray at the upper left corner and you will see a decayed wisdom tooth.The decay is the dark spot about half way up the tooth. This was the source of her pain and the removal of this tooth cured the site of the pain in her front tooth. This tooth was partially hidden by its position next to her cheek making it difficult to see the cavity, However, when the patients subjective compaint does not match your observation of the area, a more in depth exam and x ray will help discover the "root" of the problem.

Hey Doc why are my teeth getting longer and they have notches at the gum line?
What is displayed here is called recession with abfraction. Recession is the pulling away of the gum from the tooth as it receeds up the root exposing more of the tooth. Abfraction is the notching of the tooth at or just above the gum line. Both conditions can cause sensitivity to cold liquids or even when you breathe in cold air and have a "root" cause from tooth grinding known as bruxism. Treatment for these problems ranges from the simple applications of desensitizers and bite guards to more complex remedies such as having a gum graft done or root canal tretment.
If you notice your teeth becomming longer or developing notches at the gum line, consult your dentist for appropriate therapy.

My usual way to straighten teeth is through non extractions.This case shows a severely crowded dental case with constricted arch spaces leaving little room for the adult teeth. I was taught how to save all of the adult teeth and make them fit where they belong.

"But it doesn't hurt,how can i have a cavity?" Dental caries, also known as cavities begin as acid plaque erosions into the enamel of the tooth,in this case, through a small pit . Look closely and you can see what i saw on this patients tooth. The caries is spreading out under the enamel and infecting the underlying dentin of the tooth.Although small in size, about 1 milimeter, it would have continued to grow and infect the nerve of the tooth. Then the patient would feel the "hurt" from the cavity. This is why periodic exams are important to find little things before they become big problems.

The importance of regular x rays is shown here The thin black line in the middle of the teeth is the nerve canal of this patients lower front teeth. In one of the teeth this canal space is enlarged, look closely and you'll see it. Its the second tooth from the right and it has a condition known as INTERNAL RESORPTION. Left untreated the root of the tooth will completely dissolve away and the tooth will just fall out. This in one of the reasons why we as Dentists and Hygienists request x rays on a regular basis.
The second picture shows a different view, the tooth with the resorption is the second from the left. It shows a almost circular erosion of the root of the tooth.

OOPS!~ fractures sometimes happen in teeth with small to moderate sized amalgam fillings. This tooth was saved by placing a crown but other fractures can result in the loss of your tooth. If you have a sharp,fleeting pain on biting in a certain spot on your tooth, that is a hair line fracture that needs immediate attention.
