A 31-year-old man presented with painful loss of vision in his left eye for 4 days. Slit-lamp examination revealed a live worm with scolex stuck to the corneal endothelium showing torpid movements on illumination in the anterior chamber. There was no view of the fundus, and ultrasonography showed a well-defined cyst in the vitreous cavity with hyper-echoic scolex and 1 cyst in lateral re**us as well. The worm could have migrated from the vitreous cavity, angle, or from surrounding ciliary body vasculature. This image illustrates ocular manifestations of cysticercosis.
