Considering CareHarmony? Here is what you need to know before enrolling.

If you’re a Medicare patient living with two or more chronic conditions, you may be eligible for CareHarmony—a program designed to give you extra support between doctor visits.

After enrolling, a care coordinator will reach out by phone to introduce themselves, learn about your health goals, and begin creating your customized care plan.

🩺 Costs and Coverage:
👉 If you're eligible, CareHarmony is covered by Medicare, though costs may vary depending on your insurance.
👉 If you have secondary insurance, you may have no out-of-pocket costs.
👉 Without secondary insurance, you may pay a monthly copay (typically 20%), subject to your deductible and coinsurance.
👉 It’s always best to review your specific plan.
👉 You can opt out anytime—no contracts, no commitments.

CareHarmony is already an active service, and you can enroll at any time. Beginning in the new year, Oaklawn will also be reaching out to patients who may qualify.
