Our Hospital to Home program serves as a portal to Medicaid home-based services such as home-delivered meals, personal care services, homemaker services, and more. If you or a family member find yourselves admitted to the hospital and feel you would benefit from assistance in your home, ask the hospital staff about referring you to the Hospital to Home program.

Hospital to Home is a fast-track, fast-moving process and requires an in-person visit at the hospital with the patient as well as a home assessment arranged with a family member or friend before the patient is discharged home. If the hospital patient (Full Medicaid recipient) is approved, they would be eligible for services upon their discharge home in most situations. You may have Medicare as well as Medicaid, but you must have Full Medicaid to be eligible.
