We offer healthcare access, resources & support for our neighbors to lead their healthiest lives.

The Grand County Rural Health Network is a nonprofit organization that services both Grand and Jackson counties in Colorado. We believe everyone has the right to health care and to understand how to access that care. We understand that good health encompasses more than health care, and that we are all healthier together. Our dedicated staff works in partnership with health providers, businesses, and community members to improve the future of health systems in our community. We work to educate our neighbors and help them navigate those systems to ensure they have the information and services they need to live their healthiest lives.

Tonight, Feb. 19th, is our Community IMPACT team meeting, 6-8 p.m. at the Grand County Board of Realtors office (45 N. Second St., Granby). Spanish speakers are encouraged to attend online for simultaneous English/Spanish interpretation. We will provide dinner at the meeting, and we can also pay you for your time and transportation costs if you qualify financially!

Ever wondered about the Community IMPACT Team but felt a little shy about jumping into an in-person meeting right away? We totally get it! That's why we'd love for you to join us virtually and see what we're all about.

You’re invited to the Jackson County Youth Prevention Coalition Kick-Off Meeting 💙

Over the past year, many community members shared their time and insights through surveys, interviews, and conversations to help us better understand youth substance use in Jackson County. That input shaped our Community Profile and Readiness Assessment — and now it’s time to share what we learned and talk about what’s next.

At this meeting, we will:
• Share key findings from the community assessment
• Discuss Year 2 of the five-year SAMHSA prevention grant
• Outline the direction of prevention efforts this year
• Introduce the formation of the Jackson County Prevention Coalition

Attending this meeting does not require a commitment to ongoing involvement. The goal is to share information, answer questions, and invite community voice as we move forward. This work is for the community and by the community.

We hope you’ll join us to help shape what comes next for Jackson County youth.

The Community IMPACT Team gives a huge thank you to Middle Park Stock Growers Association and CSU Extension Grand County for making the January 30th screening of “Legacy” such a success!

The film screening was the latest event in the Light in the Valley series and highlights the importance of mental health, connection, and support within the agricultural community.

Make plans to stop by Hideaway Park Brewery this Wednesday from 5–7pm for a Tap Room Meet & Greet with the Rural Health Network team.

Come say hello, learn more about our programs and services, and connect with some of the people behind the work supporting health and wellbeing in our community.

Huge thanks to the Brewery staff for supporting the Rural Health Network through their quarterly fundraiser. Now through the end of March, every visit to this community-focused local Brewery helps support our vital work!
