Grand County Rural Health Network We offer healthcare access, resources & support for our neighbors to lead their healthiest lives.

The Grand County Rural Health Network is a nonprofit organization that services both Grand and Jackson counties in Colorado. We believe everyone has the right to health care and to understand how to access that care. We understand that good health encompasses more than health care, and that we are all healthier together. Our dedicated staff works in partnership with health providers, businesses, and community members to improve the future of health systems in our community. We work to educate our neighbors and help them navigate those systems to ensure they have the information and services they need to live their healthiest lives.

Tomorrow evening, April 16th, will be our next Community IMPACT Team meeting from 6 to 8 p.m. at the Fraser Library. Spanish-speakers are encouraged to attend online for English/Spanish simultaneous interpretation.

📌 Did you know you don’t have to wait until November to get health insurance?

Even though Open Enrollment doesn’t start until November, you may be able to enroll right now if you’ve had a qualifying life change.
✅ Special Enrollment Periods (SEPs) through Connect for Health Colorado allow you to sign up for health coverage outside of Open Enrollment.
✅ Most SEPs give you 60 days from the life event to enroll.
Common qualifying events include:
• Loss of health coverage
• Marriage or divorce
• Birth or adoption
• Moving to a new area
• Other major life changes
📄 What you’ll need:
You’ll likely be asked for documentation, such as proof of lost coverage or a marriage certificate.

Aunque el Período de Inscripción Abierta no comienza hasta noviembre, es posible que pueda inscribirse ahora mismo si ha tenido un cambio de vida calificado.

✅ Los Períodos Especiales de Inscripción (SEP) a través de Connect for Health Colorado le permiten inscribirse en un seguro de salud fuera del Período de Inscripción Abierta.
✅ La mayoría de los SEP le brindan 60 días a partir del evento de vida para inscribirse.
Algunos eventos comunes que califican incluyen:
• Pérdida de cobertura de salud
• Matrimonio o divorcio

Are you in recovery? Do you want to use your life experience and skills to help others on a similar journey? Join our growing team and make a difference! As a Peer Recovery Coordinator, your voice and story matter. You’ll have the opportunity to empower others, foster substance use and mental health recovery, and contribute to a supportive team environment. This role allows you to make a meaningful difference every day by guiding individuals served toward hope, resilience, and self-determined recovery.

The Peer Recovery Coordinator provides individualized, culturally responsive non-clinical peer support, guidance, navigation, and coaching to adults and families facing struggles with substance use, including Substance Use Disorder (SUD). Collaborates with partners, including community organizations, healthcare providers, law enforcement, and judicial districts to serve community members and bridge gaps in care.

Must be eligible to complete training to become a Peer Coach through the Colorado Peer & Family Specialist certification program, which requires a minimum of 1 year in active and sustained recovery. Health or human services experience/knowledge and/or experience working with SUD population preferred. Regular travel within Grand County and monthly to Jackson County required. Ability to work some evenings and weekends preferred. Bilingual and bi-cultural are strongly encouraged to apply.

Meet our Recovery Coordination Team! Katie Moon, Recovery Coordinator, provides individual, group, and community support for those affected by substance use, and their friends and family. Stevie Kitterman was recently promoted to Prevention & Recovery Manager She started in 2023 as our first Recovery Coordinator, and has helped grow our Recovery Coordination program to what it is today. Stay tuned, as we will be posting a job opening for a new Peer Recovery Coordinator soon!

Through the program, participants are empowered with recovery resources and encouraged to stay engaged for long-term recovery. Friends and family can learn coping strategies, connect to resources and community, and understand how to support in recovery and care plans.

Our Recovery Coordinators are Certified Peer and Family Specialists (CPFS) and are trained in Trauma-Informed Care and Intentional Peer Support. This makes peers especially effective at:

✅Providing non-judgmental personal support and advocacy
✅Helping individuals know when and where to seek help
✅Supporting self-management and development of coping skills
✅Empowering an individual in recovery to maintain a healthy lifestyle
