Do you gain weight after stopping injectable semaglutide (Wegovy/Ozempic)?

In this clinical trial extension, participants on maximum dose Wegovy for 68 weeks stopped therapy and were followed for 1 year. After 1 year, these participants regained approximately two-thirds of lost body weight.

A few takeaways:
1) The chronic nature of obesity, similar to high blood pressure or high cholesterol
2) The need for comprehensive, long-term management to improve and maintain body weight and health

The American Diabetes Association (ADA) recommends testing for prediabetes or diabetes in adults without symptoms using the following criteria:

- All people beginning at age 45 years
- Adults with overweight or obesity with 1 or more risk factor (high blood pressure, low HDL cholesterol, high triglycerides, polycystic o***y syndrome (PCOS), first degree relative with diabetes, history of cardiovascular disease, etc.)
- Women diagnosed with gestational diabetes should have testing at least every 3 years
- Adults with prediabetes should be tested yearly

2 commonly used tests are fasting blood glucose and hemoglobin A1c. Each test has criteria to classify blood glucose levels as normal, prediabetes, or diabetes.

Both tests are easily done and a good starting point for assessing blood glucose control. Your primary care physician can test for the above and develop a (pre)diabetes prevention plan. Points to share:

- Thyroid autoimmunity is more common in women and with increasing age.

- Antibodies against thyroglobulin (a product of the thyroid gland) are less common and less strongly associated with thyroid dysfunction. In other words, thyroglobulin antibody has less clinical utility in the evaluation and management of autoimmune thyroid disease.

- Pregnancy increases thyroid hormone requirement. Majority of women receiving levothyroxine will require an increased dose early in the first trimester.

- Women receiving treatment during pregnancy require regular testing and close monitoring, particularly during the first half of pregnancy.

- There is an increased rate of autoimmune disease in the postpartum period due to a rebound of the immune system.

- Increasing age is associated with shifts of thyroid-stimulating hormone (TSH) to higher levels. These elevations in TSH may not represent thyroid dysfunction.

- Excessive levothyroxine doses are more common in older women than in older men. Excessive levothyroxine is associated with increased risk of abnormal heart rhythm (arrhythmia) and fracture.
