An important question that we often hear in Hospice is “When is the best time to seek/discuss hospice care? Now is the best time to learn, seek and discuss comfort care. Although end-of-life care may be difficult to discuss, it is best for family members and patients to fully understand their care options. Discussing options early allows the patient the opportunity to share their goals, priorities, and wishes. With hospice, Medicare defines four levels of care.
Hospice at Home- routine care at home aimed at increasing comfort and quality of life. Continuous Care/Crisis care- may be necessary when a patient experiences a medical crisis or when symptoms require more intensive management. Inpatient care- maybe necessary if the symptoms can longer be managed at home. Respite care- short term stays at a SNF/community.
