Our Executive Director is leading the funeral of a former patient- a beautiful soul who recently passed away. While their time under our care has ended, the impact they made on us remains.

At Concho Hearts Hospice, we believe that care goes far beyond a diagnosis. It’s about the people, the families, the stories, and the memories. We don’t stop caring when the treatments end. We walk with our patients and their loved ones through every step.

Today is a reminder that what we do is more than a job. It’s a calling. And we are honored to continue showing up for the people who have touched our lives.❤️

Concho Hearts Hospice is looking for a Full-Time RN Case Manager to join our team!

This role covers patients in the North Houston to Conroe area. We offer a competitive salary, mileage reimbursement, full benefits, and most importantly, a team that truly supports you.

As a full-time RN Case Manager, you’ll have the ability to build out your schedule because you’ll have a manageable caseload, allowing you to focus on your patient and families needs.

We’re not doing hospice the typical way. We believe in meaningful care, supportive leadership, and building something better—for patients and for our staff.

-Must be a licensed RN in Texas
-Hospice experience preferred, not required
