After careful consideration, we have made the difficult decision to close Full Circle OB/GYN, PA. This decision has not been made lightly, and we understand the impact it may have on you and your healthcare needs. We want to assure you that patient care and well-being have always been our top priority, and this decision was made with these principles in mind.
Dr. Triche is relocating to Legacy Community Health located at 3509 Garth Road, Baytown, TX 77521 and is available to see patients there. Please see the information below to obtain your medical records.
Obtaining Your Medical Records:
Ensuring continuity of care is paramount. We have made arrangements for the secure transfer of your medical records. You can obtain your records by following these steps:
• Contact Us: Please reach out to our office via email at milipt@mooemail.com.
• Transfer to New Provider: If you have chosen a new healthcare provider, please have your new provider send a medical record release by fax to 844-813-8962
Legal Rights and Protections:
Under the Health Insurance Portability and Accountability Act (HIPAA), you have the right to access your medical records. Providers must respond to requests within 30 days, with a possible extension of an additional 30 days if necessary.
