When we perform the All Me Augmentation we keep the patients in our outpatient surgical center overnight. We have nurses check the flap throughout the evening and then the next morning the surgeon checks the flap once again what we visually and check the flap of the Doppler we have a zip closure that we do which is a purse string suture that we use to close the breasts completely.

The patients will go home. We'll have them have light activity for about 2 to 3 weeks. After surgery we use xarelto postoperatively for 9 days, for both DBT prophylaxis and to make sure that we don't have any issues. If the patient has an undiagnosed clotting disorder after that the patients can resume their normal activities. Typically around 3 weeks and then they can go back to exercise and heavy lifting at about 6 weeks. We typically tell the patient to maintain a low sodium high protein diet throughout the recovery period.

The All Me Augmentation surgery is basically an abdominoplasty procedure but instead of simply taking the tissue out and removing it and discarding it you dissect out the perforated that's really the only difference in the abdomen and by using the microfascial incision where incision is between 1.2 and 2cm it's really not a big fast incision it's only in the antier sheath and therefore we're not really adding much morbidity to the abdomen.

We're taking out the tissue with the blood vessel and the perforator and because we tip we don't need to use the entirety of the tissue. You can truly customize this shape of that tissue where you transplant the breasts in order to transplant only what you really want to in the breast. We're very careful we put the tissue underneath the pectoral's fascia we do that because there are concerns about what if this patient gets breast cancer in the future. Well if we've put it dermacyte up and we put it underneath the pectoral's fascia if god forbid that patient does get cancer in the future we can go back and do a mastectomy in a truly oncologically sound manner maintaining the DIEP flap tissue on the chest wall and in essence you've already done your breast reconstruction so that's another addition that we have to talk about.

For the most part in the chest the approach depends so much on what we're doing with the chest whether it's a primary augmentation, an augmentation that does not require a lift, or an augmentation with a capsulectomy. Removal of old implants so all of those have their own little nuances that we have to consider when we're doing the All Me Augmentation and I would say the feeling of getting into the All Me Augmentation is I think every DIEP flap surgeon knows that satisfaction when you're converting a failed implant and replacing it with a DIEP flap or if somebody's having um a ni**le sparing mastectomy and you're doing an immediate reconstruction with a DIEP flap that's probably the greatest result.....

The main challenge of the All Me Augmentation is that it truly is the dividend of performing thousands of DIEP flap for breast cancer. The surgeon who's first out of practice the All Me Augmentation is probably not the best procedure for them. The reality is that it is the skills that you learn from doing hundreds if not thousands of DIEP flap fore breast cancer patients that you can then apply to the cosmetic procedure of the All Me Augmentation.

I definitely think in a cosmetic case the bar is extremely high and the minimally invasive techniques you've really described particularly going into the lower rib interspace doing a micro-facial technique. These are all things that make this a much more parable surgery for a patient and move it into the viable realm of doing a cosmetic operation using microsurgery. And and I agree Joe and I don't think that that happens overnight.

I think that you can't go and say oh I wanna have an All Me practice. What it really has to be is I've had a DIEP flat practice for many years. I've become very facile. The surgery I can do of a micro fascial incision. I don't take out ribs and then wait a second. I also do cosmetic surgery and I want to merge these two disciplines. I think that's the the reality for the All Me Augmentation is that it is truly the merger of reconstructive anti cosmetic surgery. I totally agree.

I think it's also an amazing evolution to push your comfort zone appropriately and evolve as a surgeon personally. I think ultimately the goal that we all aspire to as reconstructive surgeons is to treat every case ultimately as a cosmetic one and this is kind of the ultimate expression of that.

when I'm approaching a patient for All Me Augmentation, my first and foremost thing is to look at the abdomen. I want to start out by giving them the best abdominal aesthetic outcome that I can. This comes from years of doing tummy tuck and I don't vary the procedure one bit from an abdominal plasty and from the general approach to All Me Augmentation.

Some things that I do vary are these: I tend to look for media row perforators because those can be incorporated almost completely in the diastasis repair that I will unbearably do. The other thing I want to think about is breast volume. We've All Me Augmentation mass effects and we understand that different parts of the breast are easier to deal with a lift than others. So if a patient have an explantation I'll often volume displacer implant and know exactly what upper removing as far as implant then I can use more tissue from the abdomen and get rid of more tissue from the breast.

Knowing that, I'm going to be able to augment that breast in a higher location and give them a nicer aesthetic outcome. Ssome of the great benefits that I've seen of the All Me Augmentation are that many patients have sub muscular implants and this is obviously going to a be on top of the muscle. So patients with discomfort pain greatly benefit. But also I see All Me Augmentation as a mommy makeover that's really customizable that's really custom tailored because you can trim that tissue and suture it and shape it in any way you like.

Unlike an implant we're just hoping it'll stay where you put it and you're relying on the coverage. Whatever's covering it for your ultimate aesthetic result. In this case you can really customize you. Know here you're putting it's gonna stay and you know that over time it's only gonna look better as the patient ages and it blends unlike an implant which tends to look you know worse over time.

When I talk to a patient about the All Me Augmentation, I focus on a lot of different things including the short term, long term recovery and expectations specifically regarding recovery. I want to talk to the patients about the typical recovery from a DIEP flow.
Now we do this with a rib spraying technique in the micro fascial incision. I tell most patients that they'll get back to 90% of their activities typically around 3 weeks. The remaining 10% of activities who simply exercise and heavy lifting we want to hold off on that till about six weeks. I think Doctor Boutros has pioneered the operation and I don't think I could honestly have any more to that.
I think the patients do remarkably well. It's been an incredible, I think contribution to the field of breast aesthetics and breast reconstruction.

The best candidate for an All Me Augmentation is a patient that wants an abdominoplasty and needs a breast augmentation, or has a significant problem with her current implants including breast implant illness or capsular contracture.
In general, the Contraindications for All Me Augmentation a prior abdominoplasty or stative abdominal a hernia repair a prior surgery, and I think patients also that have had multiple procedures for their breast implants that have really not gone to where they wanna be.
Oftentimes it's because the limitation of the breast implant and the body's reaction to it and that's where All Me Augmentation has exceptional benefit.

I'm Doctor Sean Boutros. I'm a plastic surgeon in Houston Texas at myHoustonSurgeons, and I'm Doctor Joseph Diane. I'm a plastic surgeon at the Institute for Advanced Reconstruction. The All Me Augmentation is a unique approach to breast augmentation where we're basically doing a DIEP flap from the abdomen and using it to augment the breast.

The considerations that you have to think about when doing an All Me Augmentation are exactly what you would think about for a DIEP flap for breast cancer reconstruction. We have to think about recipient and donor vessels in the chest. We specifically want to approach lower blood vessels in order to preserve the sensation and the vascularity of the ni**le are complex especially if needle pedicles being used, and ultimately just as a consideration that you would have for doing a tummy tuck you want a patient who would benefit from from a doing a tummy tuck so for an All Me Augmentation.

I think Doctor Boutros has a great general guideline that if the belly button is vertical if it's high probably not the best candidate, but if that belly button is horizontal if there's skin laxity that would give them a net benefit in their abdomen they would be a good candidate and better to use that than an implant than throw it out.

I love being a surgeon. I love being able to offer my patient options for breast augmentation that simply aren't out there. It's very exciting but it all goes back to a very common poor belief. I believe that taking care of patients is an honor and a privilege.

It's an honor that someone will trust you to take care of them and it's an honor that I don't take lightly. I think that the best way that I can be a the best doctor for my patients is a simple advice I was given long time ago that is to love your patients, to treat them with love kindness and compassion and treat every patient like they're unique and special because they are.

I tell patients the best thing they can do both before and and after the All Me Augmentation is pursue a healthy lifestyle. This includes eating the right foods, a well balanced diet and exercise. Exercise is incredibly important to strengthen the abdominal muscles and strengthen the core muscles and to give the patient a good sense of well being and health.

With the All Me Augmentation there are scars both in the abdomen and on the breast. In the abdomen the scar looks almost identical to that of an abdominoplasty or tummy tuck procedure. It's a low scar, usually hidden in even the most revealing undergarments for the breast. The scars depend on what type of procedure the breast needs. If the breast simply needs an augmentation a 4 to 6 centimeter incision is typically made at the inferior fold of the breast.
Through that, the tissues inserted and the blood vessels are reconnected. However, many patients, especially patients who've had children or of breastfed will require a breast lift procedure that typically requires an incision around the ar**la and down the center of the breast and occasionally also in the fold of the breast.

The All Me Augmentation is not a minor surgery, it's a major surgery and with any major surgery there are inherent risks. Those risks include things like blood clots, strokes, and heart attacks. Thank goodness, those are incredibly rare. The most common complication of the All Me Augmentation are small areas in the abdomen which don't heal really fast. Sometimes the wounds can take a while to heal and as a result the patient may have to do some local wound cure in the abdomen.
Also, any surgery such as this is gonna have scar eat. There are scars where the tummy tuck is performed and there are scars in the breast where we place the tissue.
Anytime there's a scar it can heal almost imperceptibly or there can be a visible scar. Fortunately, we have many things we can do four scars including laser treatments and creams that can help mitigate those scars and make them blend very well over time.
Recovery for the All Me Augmentation is very similar to that of an abdominoplasty an a breast augmentation or a breast lift with an augmentation. It usually takes three weeks for the patient to get back to 90% their activities. Activities such as exercise or heavy weightlifting require an additional three weeks before the patient is clear to do those things. In general patients are free to do many things at about 2 to 3 weeks. The best thing a patient can do to speed up the recovery after surgery is be in the best shape as possible prior to surgery things like exercise and proper diets are really important to help speed the body's recovery.

The All Me augmentation is basically a tummy tuck and a breast augmentation. An abdominoplasty or tummy tuck is a procedure where the lower abdominal skin and fat is removed. However, in an All Me Augmentation we simply don't just remove that tissue, but we remove it along with the blood vessels that keep that tissue alive. We then go to the breast and augment the breast with that tissue by connecting those blood vessels from the abdominal tissue to blood vessels in the chest.

As a result that tissue has blood supply, it's alive and then we use it to augment the breast. Now the abdomen is treated just like a normal tummy tuck and the fact is we remove the lower abdominal skin and fat but we also do liposuction to shape the abdomen and we tighten the abdominal muscles in order to give a nice tight attractive waistline.
