We see it all the time, patients are surprised by how much support is actually available to them beyond skilled care!
Because there is so much confusion, we wanted to lay out what one of our most important (and least understood) team members does:
1. Assessment and Planning
- Evaluating a patient's living situation, family support system, emotional state, and financial health to identify any barriers to their recovery.
- Working directly with doctors and nurses to build a realistic treatment plan that actually fits the patient's home life and capabilities.
2. Mental Health and Counseling
- Stepping in during emergencies, such as sudden health declines, suspected elder or child abuse, or severe family conflicts regarding care.
- Providing individual or family counseling to help patients process a difficult diagnosis, trauma, or the loss of physical independence.
- Identifying when family members are experiencing severe burnout and securing respite care or support groups for them.
3. Financial and Resource Navigation
- Helping families untangle Medicare, Medicaid, and private insurance paperwork to figure out what is covered.
- Tracking down local grants, food delivery services (like Meals on Wheels), and utility assistance programs.
- Coordinating the delivery and setup of necessary durable medical equipment, like hospital beds, oxygen tanks, or wheelchairs.
4. Discharge and Transition Logistics
- Organizing the safe transition of a patient from a hospital to their house, an inpatient rehab center, or a skilled nursing facility.
- Acting as the central point of contact to ensure the primary care doctor, specialists, home health nurses, and family members are all sharing the same information.
5. Legal and Advance Directives
- Guiding families through the completion of advance directives, including Living Wills, Medical Power of Attorney, and Do Not Resuscitate (DNR) orders.
- Explaining hospice and palliative care options so families can make informed decisions about comfort care.
