When I evaluate the neck, I start with fixed anatomy.

The mandible is a fixed bony point. Below that is the hyoid, which sits just above the thyroid cartilage.

If you draw a straight line between the mandible and the hyoid, that creates a reference plane.

When the submandibular gland sits below that plane, tightening the neck alone will leave a secondary bulge. In those cases, removing a small portion of the gland is what allows a clean jawline and a smooth neck.

In this short snippet of our interview I check in with Vivian and ask the question I care about most: how does she feel?

Physically she feels great. Emotionally and mentally she feels great too, even though she is still processing the changes as her facial expressions continue to normalize.

That's an important part of recovery that can easily gets overlooked.

You can already see her movement returning as she smiles, wiggles her nose, and shows natural expression. This is a normal phase, and it is a good sign that the face is waking up and settling in!

Patients are brought here before surgery to get prepped in a calm, private setting, and they return to the same space immediately after surgery to recover.

We designed a private entrance and exit so patients can come and go discreetly if privacy is important to them.

Just beyond that is our instrument processing area and a fully equipped operating room where we safely perform procedures under local or general anesthesia.

For surgeons like me who chase the sharpest jawline and the cleanest neckline, that secondary bulge under the jaw matters. In select patients, part of that fullness is the submandibular gland.

When I talk about shaving the gland, the first fear is always saliva loss. That does not happen.

The submandibular gland is a minor salivary gland, and I am not removing it entirely. I am contouring a portion of it to restore definition. The decision is deliberate and conservative, based on anatomy and balance, not excess.

This is about refining the neck so the result looks intentional, natural, and long lasting, not overdone.

Submandibular glands sit just under the jaw on each side.

They are minor salivary glands and make only a small portion of total saliva compared with the parotid.

Why does this matter for facelift and neck lift planning? A crisp jawline and sharp neck are core goals. With age, these glands can descend or look more prominent, softening that angle and blunting definition.

During evaluation I assess the glands along with skin, fat, muscle, and bone. Sometimes deep tissue lifting and contouring are enough. Sometimes we address visible gland show to refine the outline.

Bottom line: not every heavy neck is fat. Tailor the plan to the structures you actually have for a cleaner, longer lasting result.
