When I evaluate the neck, I start with fixed anatomy.
The mandible is a fixed bony point. Below that is the hyoid, which sits just above the thyroid cartilage.
If you draw a straight line between the mandible and the hyoid, that creates a reference plane.
When the submandibular gland sits below that plane, tightening the neck alone will leave a secondary bulge. In those cases, removing a small portion of the gland is what allows a clean jawline and a smooth neck.
