Our community-based mobile outreach program is committed to helping individuals and families who are struggling with opioid use disorder and/or substance use. Our staff is on-hand to “just talk” or to help connect individuals to the many treatment options available.

National Prescription Drug Take Back Day
Make sure you gather your expired, unused, and unwanted medications and bring them on Saturday, October 28th from 10AM to 2PM to the Chatham Police Department in Tracy Hall, Village of Chatham.
If you can't make it on Saturday, October 28th, the Chatham Police Department has a Medication Drop Box located in the lobby of Tracy Hall that is available anytime the building is open or by appointment.

🚨 OVERDOSE AWARENESS ALERT 🚨

OUT OF COUNTY - Ulster County has reported 12 overdoses in the last 5 days including at least 1 fatality. Columbia county has recorded 1 non fatal overdose in this time and Greene County has recorded 1 non fatal overdose during this time.

Narcan is available from Greener Pathways by texting NARCANKIT to 21000.

Fentanyl TESTSTRIPS and Xylazine TESTSTRIPS are available by texting TESTSTRIPS to 77948.

Congratulations to Twin County Recovery Services Family Support Navigator Eleanor DeleoKinney-Shook.

Today, Eleanor received her Certified Recovery Peer Advocate - Provisional (CRPA-P) status after having completed all of the training and application requirements.

TCRS’ Greener Pathways program is proud to have Ellie as part of our team. She is available to work with families of both Columbia and Greene Counties that have a loved one impacted by a substance use disorder. Ellie also leads our Family Support Group meetings on the 2nd and 4th Wednesdays of the month at 630pm. She can be reached Monday through Friday at 518-410-1192.

This week marks 5 years since Twin County Recovery Services and Columbia Memorial Health launched their partnership to provide the hospital with the services of a Certified Recovery Peer Advocate to work with patients that had overdosed or presented to the ER with symptoms related to a substance use disorder issue.

In the beginning, a Peer Advocate from Greener Pathways was on-call to respond to the hospital as needed. The program has since expanded to have a dedicated Peer Advocate, Robert Hoyt, embedded Monday through Friday 8am to 4pm in an office at the hospital. Today, that peer advocate not only meets with patients in the ER, but patients admitted to the various floors of the hospital as well as the in-patient psychiatry unit and can see patients at the request of a primary care doctor in the Medical Office Building.

This truly lifesaving service is about meeting people where they are at and at a time they are ready to have a conversation about their substance use disorder.

From connections to community supports, referrals for outpatient treatment , Medication Assisted Treatment, Narcan and Harm Reduction tools, and inpatient treatment like detox or rehab, Robert has a full arsenal of tools at his disposal to help people address their substance use disorders.

The Certified Recovery Peer Advocate provides services to about 45 individuals monthly.

This partnership is the result of a lot of leg work and cooperation between many different people and agencies. It’s a testament to what is possible when people work together for the common good.

Our Story

Serving Greene & Columbia County, our community-based outreach program is committed to helping individuals and families struggling with all forms of substance use disorder. We are an NYS OASAS designated Center of Treatment Innovation Program. (COTI). Greener Pathways was created in 2017 when Twin County Recovery Services received funding from NYS OASAS to create a mobile outreach program in Greene County NY. This funding was awarded based on statistics stating that, per Capita, Greene County had one of the highest Opioid Overdose Death Rates in the State of NY. In April of 2018, OASAS provided additional funding to expand the Greener Pathways program to begin operating in Columbia County while also continuing to operate in Greene County.

Today, the Greener Pathways Mobile Outreach Program provides services to residents of Greene & Columbia County who are looking for assistance with substance use disorder for themselves or for a loved one. Our team consists of a Program Director, four (4) New York State Certified Recovery Peer Advocates (CRPA’s), a Licensed Mental Health Counselor (LMHC) that serves as our Mobile Clinician, and a Transportation and Outreach Coordinator. We have a fleet of 4 vehicles to provide transportation assistance across both counties. We also have a 17foot Mobile Clinic RV to travel to communities around both counties and provide in-community clinical services to people who can’t travel to a traditional treatment clinic.

We look forward to meeting you and working with you to help you find YOUR pathway to recovery. You are important. You matter and recovery is indeed possible.
