Thank you for the opportunity to have served you and many of your friends and family members for the past 42 years.
It is with mixed emotions that I inform you that I plan to retire as of December 16, 2022. I have had the pleasure of caring for treasured patients like yourself and have delivered several thousand babies, performed countless surgeries and have been dedicated to keeping my patients healthy. I feel blessed to have done so and have greatly enjoyed my practice.
I will be available for routine annual exams and gynecology visits until that time.
Your medical records will be available in the office until that date. After that, as of January 23, 2023 you may request your records from our chosen medical record custodian; Cariend. You may contact Cariend at www.cariend.com or by calling 404-991-7080.
Cariend is not a medical provider and does not handle billing, medications, or referrals for the closed practice.
Requestors should be prepared to complete the Records Release Authorization Form and provide a copy of government-issued photo ID, as required by the HIPAA laws. The fastest way to process requests for copies of medical records is to visit Cariend’s website and submit your contact information at the link below – you will then be sent an email with instructions to securely upload your authorization form and ID: https://www.cariend.com/request-records/
If you do not have access to a computer or internet, Cariend can be reached by phone at 404- 991-7080 – please note the HIPAA laws do not allow medical records to be released based on a phone or email request.
