Over the past four years, HealthPro Heritage’s outpatient Physical and Occupational Therapy clinic has built a strong and successful partnership with The Community at Sunset Wood, providing convenient, high-quality rehabilitation services directly within the independent living setting. During this time, the therapists treated 93 of 121 residents (approximately 77%), improving access to care, increasing participation, and supporting residents in maintaining independence while aging in place. Notably, 35% of these patients received therapy following hospitalization, regaining independence within the community, and several were treated successfully after multiple hospitalizations, demonstrating the program’s effectiveness in restoring functional abilities.
The therapy team works closely with community staff to provide balance and fall prevention training, ADL retraining, home safety assessments, and wellness education. Early identification of functional changes and coordinated care planning have helped reduce fall risk and prevent further hospitalizations. As a result, residents demonstrate improved confidence and increased participation in activities of their choice.
This partnership has led to measurable functional improvements and high satisfaction among residents and families, highlighting the value of integrated outpatient therapy services within a senior living environment. 😍👍
