Dear Valued Patients and Friends,
It is with both a heavy heart and excitement for the future that I am announcing my retirement from private office practice. I have been blessed to serve in the Huntsville medical community for the last twenty years, and I have made countless enduring friendships. These wonderful relationships remind me every day of the joy this work provides. This difficult decision comes after months of thought and many prayers, and it is due to both personal circumstances and a desire for a new season in life.
I understand that finding a new healthcare provider can be a daunting task, and I want to assure you that I am committed to making this transition as smooth as possible. To assist with your continuity of care, I have arranged for your medical records to be securely transferred to Advanced OB/GYN. You may transition to this practice or request the transfer of your records to the physician of your choice.
We will close the practice on August 30, 2024. The enclosed HIPAA-compliant form is necessary to release a copy of your medical records to the new physician of your choice. Upon receipt of this signed form, my office will forward a copy of your medical records to the physician you designate. After the closing date of my practice, signed medical records request can be directed to conniekane@allwomensobgyn.com. On January 1, 2025, those requests can then be directed to Advanced OB/GYN, 250 Chateau Dr SW Ste 100 Huntsville, Al 35801, phone number (256) 881-9997.
I want to take this opportunity to express my deepest gratitude for allowing me to care for you and your families. It has been a privilege and an honor to serve as your physician, and I will always cherish the relationships I have built with each and every one of my patients.
