Some people call this kind of appointment a “telehealth visit” or “virtual care.” If you are not able to come in for an in-person appointment, please call the health center to ask about switching to a telehealth visit.

Getting enough nutritious food is essential to our overall health and well-being. Yet according to The Greater Boston Food Bank, 1 in 3 Massachusetts adults reported household food insecurity. The Food Pantry at Harbor Health makes it easier for individuals and families to get the healthy food they need in the communities served by Harbor Health’s community health centers - including an emergency food access pantry at Harbor Community Health Center-Hyannis. And thanks to generous donations from organizations, businesses, and individuals, we distributed over 305,000 pounds of healthy, nutritious food in 2024 – the most distributed in our food pantry’s 30+-year history!! We wanted to express our gratitude to all of the AMAZING partners, local businesses, volunteers, and individuals who helped Harbor Health support the health and well-being of every member of our community this Fall!!❤ THANK YOU to partners like the Dana-Farber Cancer Institute Mammogram Van which made a special trip to Harbor Community Health Center-Plymouth and Cordage Park for providing parking lot space to help our patients get important breast cancer screenings…. And to local businesses and individuals like Jordan, the store manager at the Stop & Shop on Freeport Ave. in , who generously donated gravy and spices for our holiday food program…. And to volunteers like Hannah & Aly, nursing students from Emmanuel College doing clinical hours at St. Brendan's Elementary, who helped students organize a food drive for the Food Pantry at Harbor Health. We are grateful for YOU and the difference you made in the lives of our patients and clients!! HAPPY HOLIDAYS!!

The Food Pantry at Harbor Health is hosting a Holiday Grocery Bag Pick Up Day on Thursday, November 14, 10:30 a - 1:30 pm at Harbor Community Health Center - Hyannis, 735 Attucks Lane. Bags are available while supplies last for individuals and families in need. Thanks to the generous donations from The Greater Boston Food Bank, individuals, and businesses in our community, bags are packed with a variety of dry goods, canned goods, and other items for holiday meals. We will also offer SNAP application assistance if you need help figuring out if you qualify and completing an application. For more information about the pick up day, visit (617) 533-2228.

We love when the Harbor Health team comes together for our community!!❤ Last week, staff from across our locations helped pack 100+ Thanksgiving grocery bags for people in need in , , , and surrounding communities in support of our annual holiday food program. And to share some Halloween fun with patients and clients when they visited our community health centers, teams participated in a pumpkin decorating contest putting their creative creations on display.🎃 Check out the photos of their work!!

SUMMER EBT IS HERE! Summer EBT is a new program that makes it easier for low-income families with kids and teens to get nutritious food when school is out. Most students are automatically enrolled in this program or you can apply online! If you or someone in your household already receives SNAP or DTA cash benefits, DTA will put Summer EBT benefits on your existing EBT card. If no one in your home receives DTA benefits, the benefits will be paid on a new Summer EBT card in the name of the parent/guardian who completes the application. Have questions about Summer EBT?
