Imlay City Family Practice has been named one of the Top 100 practices in the entire Aledade network for blood pressure control.

This means our patients with hypertension are among the best-controlled in the country- a powerful measure that directly reduces heart attacks, strokes, and preventable hospitalizations.

This fall, our office received the Hypertension Control Award from Aledade recognizing that more than 95% of our adult patients with hypertension have their blood pressure under control.

This is one of the most impactful ways to prevent heart attacks, strokes, and unnecessary, costly hospital visits. It reflects the heart of our work:
providing high-quality, proactive care that keeps our community healthy.
