Please see below a memo received from NC Medicaid today regarding an informational opportunity for consumer directed families. Anyone is free to register and participate. This is not a required session. Footprints does not have any additional information regarding this session. The link to register is included in the information below.

Consumer Direction Training Opportunity for Beneficiaries and Families Hosted by Applied Self-Direction



Training opportunity for individuals and their families who direct their care through the consumer-directed services. You are encouraged to send to this training opportunity to CD waiver beneficiaries and their families so they can sign up and learn more about how states administer consumer direction programs. Hello Footprints families!

As you are likely aware the state is preparing for incoming winter weather moving into the Carolinas today. Forecasts suggest a mix of rain, sleet and snow will be moving across the state for most of the day today and through the evening hours which could result in power outages and dangerous road conditions. Governor Josh Stein has declared a state of emergency to ensure resources are in place to meet the needs of North Carolina residents over the next couple days.

In anticipation of the storm, it is helpful to ensure that you have all emergency kits stocked and ready. Create an emergency supply kit with essentials like blankets, flashlights, batteries, non-perishable foods and first aid items. Make a family communications plan that details what you will do in case of emergency and ensure that emergency contact information is up to date. Bring pets indoors and move other animals or livestock to sheltered areas. Fill up your car with gas. Ensure that your electronic devices are charged. Ensure that your child's necessary care supplies are collected in a convenient location in the event that you should need to evacuate quickly.

If you are in need of assistance with locating resources or your family has to leave your home as a result of the storm, please reach out to your assigned case manager..

You can obtain information about support resources in your area such as shelters, meal sites, supply distribution points and recovery assistance by dialing 2-1-1. This service is available 24 hours/day 7 days/week.

Happy New Year Footprints families! We have received the following announcement as of this morning regarding training webinar dates for consumer directed services.

Participation in an annual training webinar by the employer of record is now a requirement for all beneficiaries currently participating in or wishing to participate in consumer directed services.

We will share additional training dates when they are made available!

CME Training Announcement:

The Community Alternatives Program for Children and Disabled Adults (CAP/C and CAP/DA) is in the process of posting the 2025 Consumer Direction training schedule, which will be posted after January 6, 2025. In the interim, the following Consumer Direction training is currently open for registration:

January 23, 2025, at 9:30 am – 12 pm

The last mandatory Community Alternatives Program for Children and Disabled Adults (CAP/C and CAP/DA) consumer direction training for calendar year 2024 is planned for Wednesday, December 11, 2024 from 5:30 p.m. to 8:00 p.m.

Please inform or assist waiver participants newly interested in consumer direction or those who have not attended a training to register for this training if they plan to begin or continue consumer direction in December 2024 or early January 2025. To meet the eligibility requirements to self-direct, a consumer direction training certificate issued by NC Medicaid must be uploaded in the e-CAP system to support the approval of the plan of care.

CONSUMER DIRECTION TRAINING FOR HURRICANE HELENE FAMILIES

This training is for families who have been impacted by Hurricane Helene who may not have access to internet at home currently.

DHB is offering a training for individuals needing to complete the consumer direction webinar to access consumer directed services.

The training will enable families to participate telephonically if needed.

NOTE: THIS TRAINING IS THIS AFTERNOON FROM 3-4PM. (We apologize for the limited notice, the information was just shared with CMEs this morning)

As we work to touch base with everyone following this weekend's storm, we wanted to provide some updated contact info for families who live in the hardest hit areas. A few of our case managers have been impacted by the flooding and are experiencing power, internet and cell outages themselves. We have established an emergency contact case manager for our families in the impacted areas. The emergency contact case manager will be able to provide assistance related to your CAPC services and answer any questions that you may have in the event that you are not able to reach your assigned case manager.

Hello Footprints families!

Hurricane Helene is projected to pass through North Carolina beginning this afternoon and the impact of this storm is anticipated to last through midday tomorrow. Local weather services are predicting significant amounts of rainfall, wind gusts and increased likelihood for flooding and tornados especially in the mountain region. North Carolina is officially under a state of emergency at this time.

In anticipation of a storm, it is helpful to ensure that you have all emergency kits stocked and ready. Create an emergency supply kit with essentials like blankets, flashlights, batteries, non-perishable foods and first aid items. Good afternoon Footprints families!

As you may be aware Governor Cooper has declared a state of emergency for North Carolina as a result of Hurricane Debby (which has now been downgraded to a tropical storm) impacting North Carolinians today. We want to ensure that all families have their emergency resources in place for the impending storms which we will be receiving over the next 24-48hours. Parts of North Carolina are expected to experience significant flooding with possible power outages, water and wind damage. Please ensure that you have your emergency kits and emergency backup equipment stocked and ready should it be needed.

Please continue to tune in to your local news station for resource management and information on expected weather conditions in your area. You can visit DriveNC.gov for updates on road conditions in your area. You may also utilize ReadyNC.gov to identify other emergent resources in your area such as temporary shelters.

If you should need to leave your home to seek shelter elsewhere due to storm damage or power outage please notify your assigned case manager as to where you will be or the best way to contact you so they can follow up with you following the storm. If you need further assistance, please reach out.

For families who are ALREADY USING consumer directed services, you must complete one training session by the end of the year. There is only one training session offered per quarter. This quarter's training is in May. If you are not able to attend the May training, please register for the August or November date.

If you think you may want to transition to consumer directed services in the next few months you may also attend the training.

CONSUMER DIRECTION TRAINING REMINDER -- NEW FAMILIES ONLY

Friendly reminder that any family considering transition to consumer directed services must participate in a mandatory DHB webinar which is only offered once per month. This month's training is scheduled for tomorrow, April 17th from 9:30am to noon.

This training is intended for families who are transitioning to consumer directed services for the first time or designated representatives who will be assuming the employer of record responsibility for the first time.

Use the link below to register and you will receive a separate email back containing the link needed to sign in to the training. Once you attend you will receive a certificate of completion via email which you can forward to your assigned case manager.

A Message from Footprints Director

Good evening Footprints Families,

I am reaching out to share some updates and information about my continued vision for Footprints Case Management in 2024. The CAPC program has experienced a significant amount of change over the last year and as such there has been increased demands placed on CAPC case managers. Transition periods can often bring about opportunities for growth and renewed focus, but that is generally not without some transformation. A few members of the Footprints team have made the decision to step away from their roles over the last few months to address personal needs and we fully support them on their continued journey. Despite their departure, Footprints continues to be a strong team of some of the most knowledgeable and compassionate case managers across the state. Our administrative team is currently working on a plan to onboard new talent with a focus on ensuring that candidates selected will possess skills that complement our existing staff as well as our future growth plans.

Footprints has also made some strategic changes to our agency's service area. As part of a continued effort to ensure manageable workloads and a positive work-life balance for our staff we have closed for services in a few outlying counties where census has been consistently low over the years so that we can focus our efforts in more centralized locations and ensure case managers can spend less time traveling and more time helping our families. Those counties include Watauga, Yadkin, Lincoln, Rowan, Rockingham and Transylvania. Over the next two months we will be making some additional changes to our Greensboro service area with the closure of Guilford and Randolph county which will allow us to maximize coverage of the families being served by Footprints in the remaining Triad area.

In a recent personal social media post I mentioned that Footprints is getting smaller to focus on a plan to grow smarter. We have made some reductions to our coverage area and our current census to allow us to ensure that our case managers and management team are able to continue to provide the same level of competent, friendly customer service to families in the medically complex community which has been a part of this company's mission for the last eleven years. We will also be getting back to basics with a renewed focus on policy, training and resource building to ensure that our employees are equipped to support the changing needs of this incredibly unique population we serve. I am tremendously grateful for the continued dedication of each and every member of the Footprints team and am feeling both excited and optimistic as we embark on our next chapter as an organization.

MANDATORY CONSUMER DIRECTION TRAINING REMINDER

Just a friendly reminder that all families utilizing consumer directed services are now required to participate in a mandatory virtual refresher training webinar facilitated by DHB at least once per year. There is only one training session offered per quarter and participation is being tracked so please plan accordingly.

The Quarter 1 training session is scheduled for Wednesday, February 28th which is next week.

Families are only required to attend one training session and may attend whichever training session best meets their scheduling needs, however, DHB has asked that families attempt to complete the training prior to their next annual review period if at all possible. Families wishing to complete the training prior to their annual review period are welcome to do so.
