AVMs—arteriovenous malformations—can be large and quite dangerous to the patient if they hemorrhage. My experience is that they are not common at autopsy; I’ve only had a couple of cases in my entire career. One of the scariest things about this entity is that sometimes you don’t even know you have it until it’s too late. But the bulk of the cases produce some symptoms which allows the patient to get scanned and the entity discovered. This is an explanation of why they aren’t seen that commonly as a cause of sudden death.

The determination of death in cases just depends on the actual cause and whether or not trauma is present. Obviously, this is a very simplistic answer because there are entire books written on how to determine death in these type cases.

Not my original photo, but I saw this on Instagram (you can see the original poster for proper credit). Obviously this civil war soldier sustained a non-survivable wound, certainly an instantaneous death with this kind of damage. The projectile entered right at the right temporal bone, which can be quite thin! Only about 2 millimeters in some areas (about twice the thickness of an average thumbnail).
