So the other day I examined a patient that I had not seen for a couple of years. His complaint was his vision continues to change and he had just seen a specialist in the UK 3 weeks ago who reported to him there was nothing wrong with his eyes. After having him read the chart (which he did not see well) I looked and measured his corneas with manual instruments. It was evident immediately where the problem was. He had a condition of unknown cause but not rare and treatable. Upon asking what the specialist did, I found like so many Drs., he looked at automated printouts and a photo of the back of his eye and missed the obvious condition. Be aware that it is difficult to beat a direct view of the structures of the eye with automatic machines. They might wow you but you may be getting shorted on a quality exam. Machines have their place but not replacing an “eye” examination.

This next year should be another adventure and tough decisions. I heard on the radio that the Indiana Medicaid program is over extended. Keep in mind many have state programs because their employer did not supply one, nothing against these small companies, are going to get caught in the problem. These programs have MORE administrators than doctors and the only way they know to fix is cut the crappy payment to the people doing the care. We need less beAn counters. Doctors, including my practice, will have to make decisions on whether we can see these patients because of lack of payment. Doctors do bean counting too. We will be watching to see the states decision on cuts. Stay tuned
