Our Story

Active Day of Indianapolis Northwest has been providing medical adult day care services in the Indianapolis area since 2008. The Indy NW center has built its outstanding reputation by creating a warm and caring environment for the members we serve. Adult day health care services are focused on meeting the needs of the frail elderly client, as well as younger adults with intellectual or developmental disabilities. Our services include nursing, assistance with personal care, therapeutic activities, a nutritious hot lunch as well as a breakfast and afternoon snack, and transportation to and from the center arranged through an area provider. Each day our members enjoy a fun calendar of events including arts and crafts, games, community field trips, and local groups and entertainers who come into the center. A person-centered care plan is developed for each member based upon the individual’s needs and goals while attending the center, and the nurse is available during the entire day to administer medications and provide health care supervision. Socialization for the member and respite for the caregiver are emphasized to assist the individual to live at home and remain active in their community as long as possible. Our services are reimbursed by the Aged & Disabled Waiver, Community Integration & Habilitation Waiver, Family Support Waiver, TBI Waiver, Social Services Block Grant, CHOICE, Veterans Benefits, and Private Pay. We encourage you to drop by the center for a tour during the hours of operation, Monday through Friday 8:00 am – 5:00pm. The Indy NW center is bright, modern, and designed to accommodate members of varying ages and health care needs. In addition, we are CARF accredited.
