We'd like to give a special shout out to Shelter House - Iowa City, Abbe's Chatham Oaks, Domestic Violence Intervention Program, Caring Hands & More, and United Action for Youth. Their staff have been collaborating with our staff to place group orders from our Food Bank so that they can pick up groceries and deliver directly to their clients. By offering to deliver to their clients, these groups have reduced the amount of individual delivery requests and are helping us keep up with all the requests!

Agencies and individuals can call (319) 519-6165 between 9:00 AM – 5:00 PM Monday through Friday to request food delivery.

The Lead Direct Support Professional position is a Full-time hourly position.

This position provides direct supervision and oversight of a SCL site(s), ensuring day to day needs of individuals served are met. Works with Direct Support Professionals, SCL management, and additional stakeholder to ensure quality service delivery. This position commonly works in an unsupervised, independent work setting where the ability to problem solve and make appropriate, on the spot decisions is essential.

Minimum Qualifications: Preference of a BA or BS from an accredited college or university and/or a combination of education and experience which totals three years. A service provision background is essential. Supervisory experience is preferred.

Other Qualifications: A valid driver's license and a driving record that is acceptable from a risk management perspective is required upon hire. The ability to communicate clearly in oral and written form and perform basic computer skills is essential.

Program Assignment: Supported Community Living

Primary Responsibilities:

A. Service Provision:

1. Works with supported community living staff and management to ensure service implementation of individual service plan.

2. Works with team members to ensure medical needs of individuals are met. This includes, but is not limited to, checking in medications, ensuring timeliness of medical appointments, tracking medication passing errors, attending appointments, following up on noted medical concerns, ensuring current and accurate documentation and communication relating to the individual’s medical needs as assigned by the Service Coordinator.

3. Responsible for managing individual’s recreation schedules. Ensures activity calendars are created and transportation to activities is scheduled.

4. Responsible for creating individual’s menus. Ensures grocery lists are kept up to date, and grocery shopping is completed as needed.

5. Ensures all files at site are kept current and maintained in accordance with established standards. Takes all needed documentation and communication to the houses.

6. Provides problem-solving and mediation for individuals and their service plan teams to resolve conflicts/foster relationships relative to the needs of the individual.

8. Ensures environmental needs of the clients are met. This includes, but is not limited to, frequent inspections of the homes and equipment, coordinating repairs and finding solutions to matters that are beyond the client’s ability and/or landlord’s responsibility.

9. Ensures cleanliness of clients’ homes. This includes, but is not limited to assigning cleaning tasks to house staff, creating cleaning schedules, and inspecting houses frequently to ensure cleaning has been done.

10. Reviews all house documentation and ensures it is being completed thoroughly, correctly, and in a timely manner. Indicates what areas of documentation needs to have corrections made and contacts staff to make the corrections. Ensures corrections are completed.

11. Facilitates monthly house meetings in addition to frequent verbal and written communications with house staff.

12. Completes additional assigned duties. The size of supervision load and caseload will vary based on the specific duties assigned, which may change as agency need dictates.

B. Supervision:

1. Works with Training Coordinator to train caregivers and substitute staff on all aspects of direct service provision and implementation of the service plan.

2. Completes and delivers caregiver evaluations in collaboration with Service Coordinators within established timelines. Provides ongoing timely feedback on performance and addresses performance and conduct concerns.

3. Provides coaching/mentoring to support caregiver’s development of essential leadership skills.

Secondary Responsibilities:

Completes office paperwork, organization and/or cleaning as assigned by CHAM administrators.

Compensation/Benefits:

Competitive compensation and benefit package offered. Compensation varies based on education and experience.

• Be at least eighteen (18) years of age.
• Demonstrated ability to read, write, and follow a written Individual Service Plan/Plan of Care.
• Good verbal, written, and interpersonal skills.
• For certain assignments, a DSP is required to be a licensed driver with automobile insured in accordance with CHAM requirements and in good repair. Good driving record.

ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY
1. Performs simple procedures to maintain flexibility.
a. Range of motion exercises
b. Assistance in ambulation or exercises
2. Performs personal care activities, including but not limited to:
a. Bathing
b. Shampooing
c. Skin care/nail care
d. Oral hygiene
e. Shaving
f. Dressing
3. Performs household services essential to health care at home, including but not limited to:
a. Meal preparation/feeding
b. Laundry
c. Light housekeeping
4. Assists in the administration of medications that are ordinarily self-administered according to CHAM policy.
5. Reports any observed or reported changes in the client's condition and/or needs to the supervisor. Follows reporting guidelines in the Individual Service Plan/Care Plan.
6. Documents care provided and completes the forms required for the client's records. Completes the appropriate records to document care given and pertinent observations. Submits documentation per CHAM requirements—at least weekly.
7. Promotes personal safety and a safe environment for clients by observing infection control practices, following CHAM guidelines, and reporting unsafe situations to the Administration/ Supervisor.
8. Demonstrates safe practice in the use of equipment. Does not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs.
9. Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.
10. Maintains confidentiality in all aspects of the job.
11. Attends in-service programs to meet compliance requirements. Identifies learning needs to CHAM supervisor.
12. Performs other related duties and responsibilities as deemed necessary.

Our Hospice Support Specialists provide end of life care at the Hospice Home of Johnson County, also known as The Bird House. The Bird House is a community based nonprofit dedicated to providing compassionate, dignified end of life care by focusing on the physical, spiritual, and emotional needs of our guests and their loves ones in a peaceful, homelike environment.

Qualifications:

-Previous caregiver experience required; end of life care experience preferred.

-Experience working independently

-Must be able to work every other weekend

-Must be able to pass multiple background checks

-Able to work 12 hour shifts

There are several openings.
- full-time overnight (3 nights per week)
- full-time days (3 days per week)

Responsibilities:

- Provide companionship, personal cares(bathing, dressing, grooming, toileting), medication management, meal preparation, housekeeping/laundry assistance, positioning, transferring, and feeding as appropriate.

- Assist with care at the time of dying and death.

- Complete medication management course or equivalent.

- Work with volunteers

- Participate in orientation and continuing education for the staff.

- Work with a team of dedicated professionals to provide the best care possible to our guests.

- Maintain confidentiality on all guests.
