The PASSPORT program provides Ohio seniors with in-home services designed to help those who are at risk of nursing facility placement remain safe at home.

✔ Available in Lawrence, Jackson, Scioto, Vinton, Gallia, and Pike Counties
✔ For individuals 60 and older who qualify
✔ Care provided at home or at the home of a family member or friend
✔ Custom service plans that work alongside support from loved ones
