π¨βπΌ Introduction:
Meet John, a 79-year-old retired mechanic who lives alone in a 2 story home. He experienced a fall while in the shower and sustained a right hip fracture. He underwent a right posterior hip replacement and returned home after a 3 day hospital stay.
π The Challenge:
Following his fall, John experienced difficulties in performing everyday tasks. Simple activities like dressing himself or preparing meals became daunting challenges. After discharging home, he spent most of his time in the recliner and reported he was too scared to get in the shower because of the fear of falling.
π The Solution:
John was referred to mobile occupational therapy and he began feeling like himself again. The process looked something like this:
π Assessment:
The occupational therapist conducted a thorough assessment of Johnβs physical, cognitive, and psychosocial status. The evaluation revealed significant right leg weakness, decreased balance, a significant fear of falling, limited mobility, and challenges in performing activities of daily living (ADLs) independently.
π οΈ Intervention:
Based on the assessment findings, the occupational therapist developed a personalized treatment plan focusing on the following key areas:
π Adaptation and Retraining: The therapist collaborated with John to identify strategies for adapting his daily routines to accommodate his physical limitations. This involved practicing tasks such as dressing with adaptive equipment, bathing, and meal preparation using modified techniques to promote independence.
πΆββοΈ Functional Mobility Training: John participated in a progressive functional exercise program aimed at improving his strength, balance, and mobility. This included tailored exercises targeting lower extremity strength and practicing functional tasks such as transferring from bed to chair, walking with assistive devices, and reaching into cabinets or down to pick up the newspaper.
π₯ Fall Prevention Strategies: John received education on fall risk factors and strategies to minimize hir risk of future falls. This included environmental modifications, such as installing grab bars, testing for orthostatic hypotension, screening for vision changes, and reviewing medications.
π€ Psychosocial Support: Recognizing John's emotional distress, the therapist provided support and coping strategies to address his fears and anxieties, including training on meditation and nonpharmacological pain management techniques.
πProgress and Outcomes:
John demonstrated remarkable progress in his physical function and emotional well-being. He experienced significant improvements in his strength, balance, and mobility, allowing him to perform ADLs with greater ease and independence. John's confidence and sense of self-efficacy also improved, as evidenced by his increased engagement in social activities and hobbies.

Follow-up:
As John transitioned back to independence in his home environment, the occupational therapist provided ongoing support and monitoring to ensure continued progress and safety. Follow-up sessions focused on reinforcing learned skills, addressing any residual challenges, and getting a home program in place for continued progress.

Moreover, he learned valuable coping mechanisms to manage his anxiety and fear, enabling him to embrace life with renewed enthusiasm.
