Here’s a snap of our home health physical therapists in the Dallas Fort Worth area working with patients on improving strength and balance.

Up to 40% of older people, recently discharged from
acute care hospitals and rehabilitation facilities,
suffer from a fall within six months after discharge.

That’s where we step in! Physical therapists (PTs) play a crucial role in home health care by helping patients regain strength, mobility, and independence, all from the comfort of their own home.

Our trained home health staff can:

- Reconcile medications to help ensure old medications are not taken in error.
- Teach proper use of new medications to help avoid negative reactions.
- Assess and correct potential fall hazards in and around the home setting.
- Identify and discuss patients’ physical limitations that could contribute to falls.
- Teach patients and family members about situations that create risks for falls, such as poor lighting, cluttered walkways, and improper footwear.
