🚨 Your MRI says “rotator cuff tear.”

But is that actually the reason your shoulder hurts?

A brand-new 2026 study in JAMA Internal Medicine looked at 600+ adults (ages 41–76) who had shoulder MRIs.

👉 99% had a rotator cuff “abnormality.”

👉 96% of people WITHOUT shoulder pain had tears or tendon changes.

👉 Even many full-thickness tears were completely pain-free.**

Let that sink in.

Almost everyone over 40 has something on MRI.

That means:

An MRI finding does not automatically equal the cause of your pain.

Why This Matters

If imaging is done first, it’s easy to:

❌ Anchor on the word “tear”

❌ Assume surgery is necessary

❌ Miss the real source of symptoms

It would be a shame to undergo surgery…

and still have pain afterward

because the “tear” wasn’t the true driver.

What Else Can Cause Shoulder Pain?

Shoulder symptoms are often multifactorial:

🔹 Cervical spine referral (neck dysfunction)

🔹 Thoracic mobility restrictions

🔹 Scapular control deficits

🔹 Load management errors

🔹 Biceps tendon irritation

🔹 AC joint sensitivity

🔹 Nervous system sensitization

Pain ≠ damage.

Imaging ≠ diagnosis.

Why PT First?

✅ Test strength, motor control, and irritability

✅ Identify whether the cuff is actually the pain generator

✅ Create a plan before jumping to imaging

If you truly need imaging or a surgical consult, we’ll guide you there.

But most atraumatic shoulder pain responds extremely well to conservative care.

Rotator cuff changes are often normal age-related findings.

Your goals become our plan.

ACL injuries can happen to anyone.
