My Prairie Chart provides you with access to your health information online, anytime! With My Prairie Chart you can:
View/Cancel appointments with a provider of your choice. View, download and transmit your health information – including diagnosis, medications, lab results, care plans and other documents released by your Care Team. Submit a prescription renewal request to your Care Team. Send/Receive messages to your Care Team. Update your own information such as address, phone, health insurance, and contact(s). Our Sliding Fee Program offers reduced prices on office visits and all other services provided in the clinic to all individuals and families who qualify. Prairie Community Health has a Certified Application Counselor who is trained to answer your questions free of charge about the Marketplace, and can help you through the enrollment process.
