Insurance: Presently, Dr. Ambo and Associates only directly bills insurance for individuals covered under an Aetna Costco employee insurance plan. If you are covered by Aetna Costco employee insurance, please make sure to provide us with your Aetna ID number. For all other insurance companies we will be happy to provide you with an itemized receipt which you can use to submit a claim with your insurance company, provided that your policy covers eye examinations.
