The Swan Family Wellness Center, a member of the Faith Community Hospital Health System, brings to Jack County many valuable resources to transform your health and wellness.

Due to technical issues with our front door, we currently do not have our fingerprint 24/7 access to the wellness center. We apologize for this inconvenience. We will update you all once the door issue is fixed.
