Geriatric care management consists of the process of planning and coordinating care of the elderly to meet their long term care needs, improve their quality of life, and maintain their independence for as long as possible. It entails working with seniors and their families by combining the knowledge of physical health, the aging process, family dynamics, and public and private resources. Geriatric

care management integrates health care and psychological care with other needed services such as: housing, home care services, nutritional services, activities of daily living/socialization as well as addressing financial and legal needs. Our Services Include:

MEDICAL MANAGEMENT - Review Medical Concerns and Provide Recommendations, Ensure Implementation of Doctor Recommendations, Accompany to Doctor Appointments

RESOURCES - Identify and Arrange Local Cost-Effective Resources, Research & Identify Financial Resources

HOME CARE SERVICES - Identify and Arrange Care Services, Arrange Medical Equipment

SAFETY & SECURITY - Monitor Care Services to Ensure Physical, Emotional, and Financial Well-being of Client and Client Interest, Complete Medication Management and Review, Respond to After Hours Emergency Care Needs 24/7

FINANCIAL - Reduce Costs by Eliminating Duplication of Services, Identify Financial Concerns and Referral to Financial Management Service, Review and Assist with Monthly Bill Paying

COMMUNICATION - Accurate Communication and Coordination of Client Treatment Plans to Service Providers, Timely and Complete Communication to Family/Designated Parties

LIVING ARRANGEMENTS - Identify and Facilitate a Transition to Alternative Living Situations, Provide Direction on Social Interaction Opportunities

INSURANCE - Initiate and Monitor Timely Processing of Long Term Care Policy

LEGAL - Referral to Legal Resources to Create Advanced Directives
