Coming home after inpatient rehab can feel overwhelming, but continuing your recovery at home is a big part of your progress. Start by setting small, achievable goals. These could include activities such as walking a certain distance each day, completing your home exercise routine, or becoming more independent with daily tasks like dressing or cooking.

To track your progress, try:
✔️Writing in a notebook or rehab journal
✔️Using a checklist or calendar to log your activities
✔️Taking photos or short videos to visually track improvements

Make time to reflect on what’s getting easier and where you're still facing challenges. Noticing small wins like less fatigue, improved balance, or finishing a task without help can boost your motivation.

If you received home exercise instructions from your therapy team, stick to them and update your notes as you go. Don’t hesitate to reach out to your doctor or care coordinator if you have questions or concerns.

When Alberto Yu was rushed to the ER, he was suddenly bed-bound with weakness in his left arm and right leg. Unable to walk or care for himself, he faced two difficult months of dependence. Determined to regain his independence and adopt a healthier lifestyle, Alberto sought out physical and occupational therapy, choosing Reunion Jacksonville for its reputation, convenient location, and commitment to individualized care plans.
