After years as a banking executive, an unexpected layoff left Mrs. Johnson (name changed to protect patient confidentiality) unemployed, uninsured, and unable to afford the care and medications she relied on for her health.

She remembered meeting Jax Care Connect at a local health fair and made the call. Her Patient Health Advocate connected her to primary care at Agape Family Health, specialty care was then coordinated through WeCareJax, and we were able to schedule the screenings and treatments she urgently needed.

Without this support, her only option would have been the ER—for something as routine as a blood pressure refill and treatment for her knee. Instead, she received compassionate, preventive care and peace of mind during one of the most uncertain chapters of her life.

Our latest evaluation shows that 76% of patients avoid returning to the emergency department within a year of engaging with JaxCareConnect, saving the local hospital system and taxpayers an average of $7,155 per avoided visit (based on FY 24 claims data). That’s not just cost savings—it’s lives redirected toward preventive, consistent, and compassionate care.

We had a great time stepping out of the office and into the beautiful for our first quarterly team outing! From art-filled halls to genuine laughs and great conversations, it was the perfect way to reconnect and recharge.

A special highlight was exploring Westward Vibrations: Dance of the Exodusters - a powerful exhibition that sparked some meaningful dialogue and reflection.
