What an amazing day as a Direct Primary Care Physician!
Today was definitely an excellent example of how DPC physicians improve care coordination worldwide. Yes, I said worldwide. My day started as usual, getting to my office at 5:30 to clean, vacuum, and mop, before getting on a transatlantic conference call with one of my patients who injured his back while on vacation. Unfortunately, he ended up with a herniated disc in his lower back and requires urgent surgical intervention. So the discussion was with him and the neurosurgeon, with plans to have surgery on Monday, then back home in a couple of weeks. I had to reiterate not to lift anything greater than 5-10 pounds, and offered to unload his luggage when he got back home.
My next patient, also a herniated disc in the lumbar spine with significant nerve entrapment. I was contacted my “in person” with an orthopedic spine surgeon who’ll be seeing my patient next week Tuesday, instead of in a month through the regular channels.
A couple of patients later at 11am, I had a patient present for a preop evaluation for surgery next week Tuesday. Her EKG had a few concerns needing additional evaluation. In a traditional fee for service practice her surgery would need to be rescheduled in order to get a cardiac evaluation. I made one call to a cardiologist friend who was able to review the EKG and agreed with my concerns and recommended an echocardiogram. One more call to my source for at home echos and she’s scheduled for an echocardiogram this evening with a report over the weekend. If everything looks good I can give the all clear for surgery and fax out the clearance on Sunday so there’s no delay in care.
