ST. JOHNS COUNTY: This Wednesday (December 17, 2025) UF Health St. Johns Mobile Outreach Clinic with Wildflower Healthcare and Brooks Rehabilitation at Home Again St. Johns ๐Ÿฉบ

The UF Health St. Johns Mobile Outreach Clinic will be at Home Again St. Johns' Dining with Dignity, located at 92 S. Dixie Highway, at the corner of SR 207, behind the Winn-Dixie plaza on Wednesday, December 17th from 5:30 to 8:00 p.m.

Services offered are Wildflower Healthcare adult outreach clinic, primary care, chronic disease management and education, diagnostic testing, wound care, and cancer screening. Brooks Rehabilitation provides support.

The mobile outreach clinic intends to "improve health outcomes and quality of life for people in vulnerable populations by providing clinical care and connections to resources that help advance the physical, social, and economic health of the communities we serve.โ€

Quick Facts *The Health Planning Council provides four core functions: Healthy Communities; Happy & Healthy Places; Smart People; Vibrant Partnerships * The only health planning organization to collect, analyze, and warehouse real-time health care utilization data submitted by hospitals and nursing homes in the 7 county northeast Florida region. * Work in partnership with regional hospital planners to conduct health care utilization studies. * Experienced in cultural competent health planning models such as the World Cafeโ€™, Community Based Participatory Research (CBPR), Community Based Participatory Marketing (CBPM), Social Marketing, Empowerment Evaluation, Strategic Framework Prevention (SPF), Mobilizing for Action through Planning and Partnerships (MAPP), Planned Approach to Community Health (PATCH).
