Sulzbacher Health We provide health services for the homeless, uninsured and those whose income falls below 200% of Federal Poverty Guidelines in the Jacksonville, FL area.

The Sulzbacher Clinics provide health services for the homeless, uninsured and those whose income falls below 200% of current Federal Poverty Guidelines in downtown Jacksonville and those within the 32233, 32250, 32082, 32224 and 32266 area codes of Jacksonville Beach. The Sulzbacher Clinics offer primary health, oral health and behavioral health services, including substance abuse counseling (in conjunction with Gateway). We provide an integrated approach to ensure the highest quality of comprehensive care for our patients. We also operate an onsite pharmacy, and participate in the various Prescription Assistance Programs offered by drug companies. The Sulzbacher Clinics are part of the Sulzbacher Center, Northeast Florida’s largest provider of comprehensive services for homeless men, women and children, and one of only a few centers in the nation to offer a full range of services for the homeless 24 hours a day, 7 days a week, 365 days a year.
