RN Case Manager - Full Time - Hultquist Place
Job Description
Hultquist Place Assisted Living facility is seeking a full time RN Case Manager to provide case management services as necessary and to support our residents in maintaining independence of function and personal choice. The ideal candidate must be a team player, be self-directed, and possess organizational, superior communication, and leadership skills.
The RN Case Manager is a Clinical Coordinator for the Assisted Living Facility and is responsible for taking administrative call for Hultquist Place.
Qualifications: Current NYS RN license. Minimum of 1 full year experience. BSN preferred.
Salary Range: $57,000- $78,901 annually, commensurate with experience
Responsibilities:
Displays competence with electronic medical record
Knowledgeable of admission and retention standards
Responsible for admission pre-assessment screening, initial nursing assessments and assessment of individuals social and functional needs and programs ability to meet resident needs.
Responsible for reassessment every 6 months or as needed to respond to changes in condition and updating of all case management assessments.
Coordinate hospital/nursing home returns with social worker/case management. Complete UAS and updates case management assessments.
Implements residence’s policies and procedures which creates a case management system that provides for initial and ongoing assessment of a resident’s strengths and needs.
Completes educational modules for UAS competence. Completes the UAS these as assigned/scheduled and in a timely, accurate fashion.
Provides or arranges for the provision of identified services, monitors the receipt of such services, evaluates effectiveness in improving resident functioning and quality of life and documents the activities and their outcomes.
Assures residents are aware of their rights and works to promote and protect these for all residents
Identifies and proposes solutions to problems that will improve information flow.
Participates in team, safety and QI meetings as assigned.
Develops, in cooperation with the Administrator, systems to organize and track resident data.
Works with residents to identify strengths and unmet needs.
Sets challenging goals for self and others. Facilitates the team in achievement of those goals.
Strengthens informal relationships among residents.
Keeps confidential resident records as indicated.
Provides assistance and support to resident’s families through regular contact with families.
Upon admission of resident, orients each new resident/family to Hultquist Place by acquainting him/her with staff, residents, rules and regulations of the facility by informing residents and/or family representatives of the Resident’s Rights, Admission Agreement, Advanced Directives, etc.
Facilitates room changes/roommate changes with residents, families and other departments.
Develops formal mechanism of communication with staff, who serve the resident to identify abrupt or progressive changes in behavior or appearance which may signify need for change in plan of care.
Assists residents and/or family with financial and health insurance concerns, and when appropriate, arranges for contact with community resources to resolve financial matters
Utilizes community resources to help manage resident’s personal and social problems i.e. blind association, mental health needs.
Maintains an up-to-date file on health and welfare services in the community.
Maintains contact with the physician when discharge planning is indicated to arrange for appropriate discharge planning and to coordinate plans with resident, family, staff and appropriate community agencies
Report complex situations to the Administrator
Identifies Residents who no longer meet retention criteria and works with Administrator, Medical Doctor, Residents and Family to move to appropriate level of care thru the discharge process
Maintains ability to assist with medication. Audited once a year by Administrator.
Participates, if requested by resident, in the organization and ongoing functioning of residents and family council.
Assists in management of Resident Complaints
Assumes other responsibilities as designated by the Director of Clinical Services; Charge, clinic management, DM clinic etc., med pass.
Assists staff development training
Attends mandatory training and participates in presenting Residents Rights, confidentiality and Advanced Directives.
Manages bereavement counseling for roommates, families and other affected individuals
Takes Administrative call for Hultquist Place on a rotation basis
Assures coverage of personnel on units by covering shifts as needed
Completes Resident chart audits
Ensures monthly weight reports are accurately maintained and monitors changes of 5lbs or more. Notifies attending physician of significant weight fluctuations and oversees compliance with reporting protocols to maintain timely and effective communication of resident health status.
Covers FSBS clinic when charge nurse is not here
Uses fax, phone, computer, copier, overhead, calculator
Safety – Follows required safety procedures
In-services – Attends all required in-service training and assures assigned staff are also up to date
