In early 2017 I retired from the VHA after 45 years spent practicing Nephrology in the US Air Force, the DOD-GS, and the VHA. Over those years, I cared for thousands of dialysis patients, with both acute and chronic kidney failure. Many required dialysis either temporarily or permanently with a plastic catheter inser

ted into their upper chest or neck. This lifeline for dialysis made it possible for them to get life-saving treatments, but it also exposed them to risks, including infection, accidental removal, or disconnection from the bloodlines with resulting blood loss or entry of air into the catheter and dialysis circuit. I created HD CLEAN LLC in mid-2018 to serve as a platform for finding ways to reduce the unavoidable risks of such treatments. I am currently partnering with dialysis providers and staffs, medical device experts, and legal and regulatory mentors to bring the ideas that result from conversations about those risks as devices to market.

Version 4.0 of the Hickman Pods is now under prototype production at PROTOLABS of Maple Plain MN. The port dimensions to hold the line from the central venous catheter hub to the connector with the dialysis machine bloodlines has been re-engineered to fit catheters having smaller tubing diameters. It's crucial to have a close but not overly-tight fit with the blood tubing so microbial contamination is prevented from getting into contact with the connector. that close contact also gently holds the tubing so line disconnection is less likely. These adaptations will make the Hickman Pod even more effective in enclosing, protecting and securing the vital dialysis bloodline-to-catheter connection point, increasing patient safety on treatment. After all, "Making Hemodialysis Safer" is the driving intent of HD CLEAN!

Following an amazing patient focus group sponsored by the American Association of Kidney Patients (AAKP) where over a dozen people with dialysis experience reviewed the Hickman Pod idea and gave favorable and valuable suggestions, I've submitted a pre-application for consideration for an NIH grant.

I've also re-engineered one of the ports in the Pod specifically for the Air Force Critical Care Airlift Team's potential use. This amazing military unit travels the world in C-17 transport aircraft to bring injured Servicemembers back to Brooke Army Medical Center for definitive care. They have developed the expertise for doing continuous dialysis in-flight during 20-plus-hour missions with mid-air refueling while performing life-saving dialysis the whole way. Astounding!

In May I'll be applying again for an AFWERX Small Business Innovation Research contract. I've had some outstanding feedback from RPC-Rabrenco, a distributor of dialysis-related supplies, and from Teleflex which cleared me to purchase some of the specific types of catheters used by the CCAT so I could refine the engineering.

As you can imagine, progress is slow when you are a one-person enterprise trying to make an improvement in an industry as big as the dialysis space. However, progress is happening!
