Our Mission is to assist individuals with disabilities in meeting their basic needs, enhancing their quality of life, & promoting their independence, while also allowing them to be active participants in the decision-making and direction of their services We have a long history of serving individuals with disabilities. Since 1955, AUCP staff have been working to ensure that individuals with disabilities and their families have access to the services and supports they need to enhance their quality of life. We have 3 office locations (Johnstown, Pittsburgh, Altoona) throughout Pennsylvania and are always looking for more participants to serve. Pittsburgh Office:

🚨 WE ARE GROWING — JOIN OUR TEAM! 🚨

Alleghenies United Cerebral Palsy (AUCP) is hiring Service Coordinators (SC) to support individuals with disabilities across Pennsylvania.

Founded in 1955, AUCP began as a grassroots effort by parents advocating for their loved ones. Today, we proudly support over 4,000 individuals throughout PA—helping people live with dignity, independence, and choice.

💙 About the Role
As a Service Coordinator, you’ll work directly with individuals and their support teams to develop and manage person-centered service plans, coordinate services, and ensure people receive the supports they need to thrive.

📍 Work Location:
Hybrid / Remote within Pennsylvania
(Community-based visits required — must live in PA or be willing to travel)

🛠 What You’ll Do:
✔️ Develop and update Individualized Service Plans (ISP)
✔️ Coordinate services with providers and community resources
✔️ Advocate for participant goals, choice, and independence
✔️ Ensure compliance with program regulations and documentation standards
✔️ Travel within your assigned region to meet with participants

🎓 Qualifications:
✔️ Bachelor’s degree in Social Work, Psychology, Education, Human Services, or related field
✔️ Experience supporting individuals with disabilities preferred
✔️ Knowledge of Medicaid Waivers, HCBS, or LTSS a plus
✔️ Strong communication, organization, and teamwork skills
