Staying Abreast Lactation Team LLC Providing evidence-based breastfeeding education & assistance in a calm and mom-goal led environment. My name is Nicole Halling. So why Private Practice??

I received my BSN through MWSU in St. Joseph, MO and have been a nurse since 2004. I started my career at The University of Kansas Hospital in 2004 as a High-Risk antepartum/mother-baby nurse. Through the years I developed a passion with the education and breastfeeding support aspect of my nursing role. I decided to pursue this passion and in 2014 I sat for and passed boards to become an International Board Certified Lactation Conultant (IBCLC). I am now an IBCLC at Mosaic Life Care and absolutely love the ability to assist families right after delivery!! It brings me great joy to be able to have a small hand in helping breastfeeding families achieve their individual breastfeeding goals!! It is my vision to provide breastfeeding education or assistance in a calm, supportive, and uplifting environment. When it comes to breastfeeding, one thing I have learned along the way is that it is detective work! It is so rewarding, but that does not mean that it is always easy. When a mother hits a speed bump in her journey, more times than not, there are many layers to the concern or problem that the mother/baby dyad are experiencing. Sometimes it is trial and error and it is likely not going to be a quick fix. You will need continued support and follow-up or check-ins to see how the plan is coming along. MY goal is to help you reach YOUR goal. It is my hope that you will find that help and support through Staying Abreast Lactation Team LLC @ The Salt Boutique. I see a need in our community and surrounding area for support when breastfeeding is not going as planned. Or for those speed bumps in life along a mother's breastfeeding journey. Although mothers can attend our breastfeeding support group at Mosaic Life care, which I absolutely love and highly recommend that all breastfeeding mothers attend, there needs to be a place for mothers to receive one-on-one, individualized care by an IBCLC. Studies show that mothers are more successful with breastfeeding if they have professional support from an IBCLC. IBCLCs go through a rigorous educational program. Many IBCLCs will have specialized training in complex breastfeeding issues through their RN background and bedside experience. IBCLCs have to pass a comprehensive exam before being granted certification through the International Board of Lactation Consultant Examiners. Most importantly, IBCLCs have to stay up to date in our practice as we must renew our license every 5 years. It is a must to stay up to date with the latest research to best help our clients! I want to be that "place" for breastfeeding families to come and receive support. Who is on the team?? Where is this Boutique?? The name of my practice is visionary. I would LOVE to partner with other IBCLCs as the need arises. I would also like to offer education and bedside hours to those pursuing their IBCLC. Eventually, I'd love to have an office space & offer breastfeeding supplies as well!

Happy World Breastfeeding Week! This year's theme is "Prioritize Breastfeeding: Create Sustainable Support Systems."

Those that know me, know firsthand how hard I struggled with breastfeeding all 4 of my children. I sought out help each time, desperate for answers as to why it wasn't working. There were no dedicated IBCLCs bedside at that time. As an OB nurse, I didn't know, my co-workers didn't know, my OB docs and Pediatricians didn't know why it wasn't working either-nor did they have the time to help me. Each time, I desired for it it work and each time, no answers. BUT...I learned a little more each time and the passion to help others was ignited. By my 4th time, with more education, support and knowledge under my belt, I was able to meet not 1 but 2 goals: to breastfeed for a year and become an International Board Certified Lactation Consultant (IBCLC). My support system was amazing and meant well and wanted the best for me, so please don't misunderstand me when I say this, but in the end...it was not my nurses, doctors, or family that helped me find the answers...it was Jenny Walters, a fellow IBCLC, who cared enough to come alongside me & without having to say hardly anything, she knew my heart and my strong desire to make this work and meet MY goal. All of this is to say that our healthcare systems can and should do better! Breastfeeding is important to so many of my patients but is so easily dismissed and unsupported when there are struggles. I can tell you that as a nurse and mother myself, our providers, nurses and doctors, are not fully equipped to provide the necessary support. As a nurse, I didn't know how to help my patients and didn't know what I didn't know! Now, as an IBCLC working inpatient and private practice, I can tell you it's not enough! I'm telling you, looking a mom in the face, who is struggling but wants a chance to breastfeed her baby, and saying "fed is best" or "sometimes breastfeeding just doesn't work" or "I formula fed and my kids are just fine" or "it doesn't matter how you feed you baby" oh, I could go on and on with all the things I was told along the way with each child I had, and each time....IT WAS NOT HELPFUL. Many moms do not have the same desire as me, and that is completely fine, this post is not about them. I'm here to tell you that there are ALOT of moms like me. I work with them everyday. They need a voice. They need the knowledge and support. AND they need it from the first latch and beyond! The IBCLC is and should be an integral piece of that mother's healthcare team.

This has been a LOOOONG process, but we are excited to announce that we have been accepted as in-network providers with Aetna! This means we can now see clients with Aetna and they no longer need to pay out of pocket and seek reimbursement for their lactation visits!

Now, in addition to BCBS and Cigna, we can see clients in-home or in-office at no out-of-pocket cost! Most insurance companies include 6 covered visits allowing that much needed post-partum follow-up care to help with latch, engorgement, weight checks and hands-on help!

There can be several reasons that a mother may struggle to produce enough milk to exclusively feed to her baby.

Common causes of Primary Lactation Failure include conditions of the breast (insufficient mammary tissue, surgery, hormonal influence on growth, etc) as well as Sheehan syndrome from severe post-partum bleeding, PCOS, theca-lutein cysts (secrete Testosterone and suppress onset of milk) etc.

Common causes of Secondary Lactation Failure include conditions in the infant that prevent them from suckling or removing milk from the breast properly (prematurity, oral restrictions, palate issues, heart conditions etc). Also any reason milk is not being emptied from the breast frequently/effectively (improper latch, timed feedings, delayed feedings, non-medical supplements, infrequent latch or emptying etc) or use of certain medications by the mother may also lead to insufficient milk volumes.

Some maternal conditions caused by hormonal imbalances may delay the onset of mature milk i.e. milk "coming in." Other causes that could result in a delay relate to the conditons and mode of delivery. Sometimes this results in a separation of mom and baby and thus a delayed initiation of breastfeeding or breast stimulation.

So what does all of this mean?? 🤔

Well, momma, it means that there is a lot that goes into figuring out the root cause of low milk supply or lactation failure. I was told almost all of the things on those slides as I painfully struggled to make enough milk for all 4 of my babies. Nobody knew how to help me, what was wrong, or cared to help me investigate. Thankfully the past 8-10 years have been full of research and support in regards to breastfeeding and breastmilk!

If you have or are struggling with milk supply then keep asking questions and find someone that will listen to your concerns and help you get to the root of it. It may be an easy fix, but if not, finding the cause will help you make peace with not being able to make enough milk.

Never forget....it does not have to be all or nothing! Figure out what works for you, find a good support system, and don't let anyone discourage you regarding your feeding choice. ❤
