MEETING THE NEED FOR SKILLED THERAPY

As the face of healthcare changes so should our perspective. Why are your patients receiving therapy? If your plan of care is not based on functional skill, then how will your patient’s status change? For years the focus has been on exercise, but that alone and without direction is not skilled treatment. In light of the fact that hospitals are now pushing patients out at an alarming rate, these patients require skilled treatment with tailored programs and protocols that dictate how the services are to be delivered. This is very important to make sure your patients are receiving the skilled care they need. Are your therapy companies focused on function or exercise and manual therapy only?

Medicare pays for skilled functional therapy. Consider your Nursing OASIS tool. large section of the OASIS is based on function however with the focus of treatment being largely exercise the very skills we are to assess and treat, are being ignored. We at Houston Occupational and Physical Therapy PLLC are functionally based in all our treatment disciplines. All exercise must be a precursor to the instruction of a functional skill by the therapist to the patient. Exercise is necessary to build a foundation of strength and ROM to enable the individual to compensate for their environment, but patients do not automatically improve their functional status from exercise only. We at Houston OT have trained each of our therapists on our functional based programs that are tailored to the individual diagnosis thereby ensuring a solid unified approach to patient treatment protocols

The problem with most therapy sessions is that the therapist too often is trained by another therapist to focus on exercise or manual therapy with no real skill taught to the patient. HOT PT has developed function-based training and documentation guidelines that all of our therapists have been taught. They have read and signed an addendum to our contract about the new rules for therapists to meet the demands of effectively treating the patient in a shorter period of time. We have specific treatment guidelines that we have made mandatory for all of our therapists. We are proud to say we do not have nor will have denials on our patients because we can justify our treatment in a professional manner using percentages instead of broad-based terminology, demonstrating incremental changes in function. Our company is a professional team of therapists. We work honestly and ethnically to ensure the best practices are being met for our patients. The focus is “Function and Skilled Therapy” so work with companies who put the patient’s functional status first.

How to grow your home health agency during a low Census

A low census can be an opportunity to create new relationships, improve processes, revise strategies, and prepare for future high-volume seasons. Here are some ways to make the most of a low census.
• Identify bottlenecks A downtime can highlight operational inefficiencies with your Company’s internal process that might otherwise go unnoticed and require immediate revision to create a smoother internal process and to increase efficiency and productivity. This can help you prepare for the next high-volume season and manage it more effectively when work picks up again.
• Monitor and improve staff performance Review and monitor compliancy standards and clearly convey agency expectations in clinician performance. Provide educational enrichment where lacking. Communicate across all departments; it's important to discuss agency policy so all staff know how to handle important situations.
• Read industry news Reading industry news and sharing educational materials can help your staff progress in their profession and stay focused on new rules and techniques in patient care. This facilitates the appropriate course in which to grow your business.
• Project development A low census is a time to reach out into the business industry creating new business relationships while exploring and developing new areas of coverage. Customizing appropriate clinical programs will be of interest to your new target audience and appropriate referral sources. For example, by providing a cardiac rehab program to the patients of your local cardiologist, you can insure a relevant and necessary form of treatment for patients with that diagnosis while increasing patient referral.
• Reflect on the past year Take stock of your agency’s achievements and recognize key staff who have devoted themselves to their work and your company. These are the relationships worth celebrating!! Rest!
• Plan ahead Review your workload and goals for the year ahead. If you make sure your agency is rooted in clinical expertise…. Financial success will surely follow.
