Big changes are happening next year in the world of coding- specifically for office and outpatient visits. Beginning at the first of the New Year (2021), how we code for new and established patients will no longer include the history and exam. Today, we are using the history and exam as a key part in determining the level of E/M that we should bill for, but that will soon be a thing of the past.

Beginning on January 1, 2021, we will now base our selection of the E/M code on the total time spent on the patient or the medical decision making (MDM) level, to which both time and MDM guidelines have updated changes taking effect in 2021.

A high-level summary of those charges is:

You must meet or surpass 2 of the 3 components in order to qualify for the MDM level, which will be based on 1) quantity and complexity of the problems addressed 2) amount and/or complexity of data reviewed and analyzed 3) risk of complications and/or morbidity or mortality
Total time will now be defined as the total time spent on that patient, on that date of service and not just face-to-face time with the physician
These coding changes apply to 99202-99215.

Practice doing notes now with the new implementations
Customize your templates to focus on getting the information needed for MDM
Check with your EHR vendor as they may already be making changes
Keep learning! Knowledge is everything.
Familiarize yourself with what is required to be used in scoring MDM
