Responsible for providing clinical case management services to court involved individuals who are diagnosed with a substance use disorder, co-occurring disorder and/or severe and persistent mental health disorder. The clinical case manager conducts comprehensive bio-psychosocial assessments and develops clinically appropriate individualized treatment service plans. Links and refers clients to necessary and effective resources while monitoring ongoing need. Uses evidence-based tools; applies relationship-building, strength-based counseling, motivational interviewing, risk-prioritization, and other related skills. Provides verbal and written reports to the courts on an as needed basis. Works collaboratively with treatment providers, family members, judicial services and other community support providers throughout the case management process.
Skills/Experience Required:
Applied knowledge and experience working with individuals suffering from severe and persistent mental illness, co-occurring disorders and substance use disorders. Demonstrated knowledge in the field of clinical counseling, psychology or social work. Training in acute crisis intervention, su***de risk assessment and ASAM criteria. Effective written and verbal communication skills. Must have completion of a Master’s degree in clinical mental health, clinical psychology, substance use treatment, social work, or related human services field. MLADC, LCMHC or LICSW preferred. Three to five years of related work experience or equivalent combination of education and experience preferred. Knowledge of the judicial and corrections systems preferred.
